Acute pericarditis

Answer - acute pericarditis

A 28-year-old woman attends the Emergency Department (ED) complaining of pleuritic chest pain. The pain is central and retrosternal and is relieved by sitting forward. She has recently returned from a trip to Australia and has a history of viral infection and flu-like symptoms. There appears to be a friction rub on auscultation.
Blood testing reveals normal D-dimers.
His electrocardiogram (ECG) reveals saddle-shaped ST elevation, and is shown below.

What diagnosis fits best with this clinical picture?

- Acute pericarditis

- Pulmonary embolus

- Myocarditis

အဖြေအားလုံးမှန်ကြပါတယ် scenario မှာလည်း viral infection followed by chest pain ဆိုတော့မြင်သာပါတယ်

ECG findings မှာ pericarditis တွက် specific ဖြစ်တာကိုကြည့်တတ်ရင်ပါပြီ

👉PR segment depression in lead 2 , PR segment elevation in aVR ဆို pericariditis ပါပဲ

ထပ်ပြီးကြည့်မယ်ဆိုရင် Spodick’s sign ကိုလည်းကြည့်လို့ရပါတယ်
👉downsloping TP segment in lead 2, V4-V6 လည်းကြည့်လို့ရပါတယ် (but not specific)
