최근 AC Immune과 Roche사의 semorinemab이 알츠하이머병 임상시험에서 실패했다는 슬픈 소식이 전해졌다. Biogen과 Abbive가 개발하던 타우 항체들도 진행성 핵상 마비 (PSP) 임상시험에서 실패했다는 작년 뉴스에 더불어 실망스러운 소식이다. 그러나 우리에게는 실망만 하고 있을 여유는 없다. 따라서 이를 교훈 삼아 앞으로의 알츠하이머병과의 전쟁에 어떻게 대처해 나아갈지 고민해야 할 것이다.
주목할 점은 이 세가지의 항체들은 모두 타우의 N-terminus를 타겟한다는 것이다. 이 N-terminus를 타겟하는 전략은 사실 2년 전부터 의문이 제기되기는 했었다. 치매 학회에서 다른 에피토프를 타겟해야 타우 전파를 억제하는 효과가 크다는 것이 보고된 것이다.
또다른 주목할 점은 이 항체들은 모두 정상 형태의 타우를 타겟한다는 것이다. 아직도 타우의 정상적인 생리 기능에 대해 모르는 것이 너무나도 많다. 그런데 알츠하이머병, PSP 등 질병 상태에서 특이적으로 증가되는 형태의 타우가 아닌, 정상 형태의 타우를 타겟하는 전략은 예기치 않은 부작용을 일으키거나 치료 효과를 상쇄시킬지 모른다.
그러므로 여러 종류의 타우 단백질들 중에서도 질병 특이적이며 병리 전파에 핵심 역할을 하는 형태의 타우를 규명하여야 할 것이고, 그 타우를 타겟하는 항체가 가장 효과적일 것으로 기대할 수 있다. (주)아델은 280번째 라이신이 아세틸화된 타우가 병리 전파에 핵심적인 형태의 타우임을 발견하였고, 이를 타겟하는 항체를 개발하여 임상시험 중인 다른 항체들과 비교하여 더 우수한 효과를 보임을 관찰하였다.
실제 알츠하이머병 환자 뇌조직을 활용하여 타우 병리 전파를 측정하는 FRET 실험을 수행하여 아델의 항체와 타사 항체를 비교한 결과, 아델의 항체가 더 우수한 억제 효과를 보이는 것이 확인되었다. 이외에도 타우 응집 억제와 미세아교세포의 타우 포식 증가 측면에서 더 우수한 효과를 확인하였고, in vivo 생쥐 모델에서도 우수한 효과를 관찰하였다.
(주)아델의 연구진들은 치매로 고통받는 환자들과 보호자들을 생각하며 우리의 전략이 부디 틀리지 않기를 기도하며 내년말에 임상1상 신청을 목표로 열심히 정진하고 있다.

Recently, sad news came that AC Immune and Roche's semorinemab had failed a clinical trial for Alzheimer's disease. This is disappointing news, along with last year's news that the tau antibodies developed by Biogen and Abbive also failed in progressive supranuclear palsy (PSP) clinical trials. But we cannot afford to be disappointed. Therefore, we must use this as a lesson to consider how to cope with the future war against Alzheimer's disease.
Note that all three antibodies target Tau's N-terminus. The strategy for targeting this N-terminus was questioned two years ago. In the Alzheimer’s conference, it has been reported that targeting other epitopes has a great effect on inhibiting the propagation of tau.
Another notable point is that all these antibodies target the normal form of tau. There is so much that we still do not know about the normal physiological function of tau. However, a strategy that targets tau in the normal form rather than the form of tau that is specifically increased in disease states such as Alzheimer's disease and PSP may cause unexpected side effects or offset the therapeutic effect.
Therefore, it is necessary to identify a form of tau that is disease-specific and plays a key role in pathological transmission among various types of tau proteins, and antibodies targeting this pathogenic tau can be expected to be the most effective. ADEL, Inc. found that the 280th lysine acetylated tau was a key form of pathology transmission, and developed an antibody targeting it and observed that it showed a better effect compared to other antibodies in clinical trials. .
As a result of comparing ADEL's tau antibody with other companies' tau antibodies by conducting a FRET experiment to measure the transmission of tau pathology using brain tissue of a real Alzheimer's disease patient, it was confirmed that ADEL's tau antibody showed a better inhibitory effect. In addition, a better effect was confirmed in terms of inhibiting tau aggregation and increasing tau uptake by microglia, and an excellent effect was also observed in an in vivo mouse model.
