πŸ’‰ SUGAMMADEX: 2–4–16 mg/kg β€” Can you remember what each dose means??
The 2/4/16 rule
One of the most useful reversal agents in modern anaesthesia πŸ‘‡

🟒 2 mg/kg β†’ Moderate block
🟠 4 mg/kg β†’ Deep block
πŸ”΄ 16 mg/kg β†’ Immediate reversal

🧠 High-yield pearl:
Sugammadex encapsulates rocuronium & vecuronium β€” but it does NOT reverse atracurium or cisatracurium.

πŸ“Œ Save this for your next anaesthesia revision.

EPIDURAL TEST DOSE πŸ’‰πŸ§  | Anesthesia Quick Revision

Before giving the full epidural dose, a properly performed test dose can help identify inadvertent intravascular or intrathecal catheter placement.

πŸ“Œ Key points to remember: β€’ Lidocaine + epinephrine test dose
β€’ Adult volume: 3 mL
β€’ Look for tachycardia β†’ possible intravascular placement
β€’ Rapid spinal block β†’ possible intrathecal placement
β€’ Always aspirate + administer incrementally
β€’ Observe closely before proceeding with the full dose

⚠️ Important: A negative test dose does not completely exclude malposition. Clinical assessment, aspiration and incremental dosing remain essential.

πŸ“š Based on principles discussed in Miller’s Anesthesia – Neuraxial Anesthesia

🚨 Important Advisory for the Anaesthesia Community 🚨

Patient safety always comes first.

The Indian Society of Anaesthesiologists (ISA) National has issued an urgent advisory recommending the immediate discontinuation of Themis-manufactured Bupivacaine for spinal anaesthesia and other clinical uses until further notice, following reports of serious adverse events currently under investigation.

πŸ”Ή Stop using the concerned product immediately.
πŸ”Ή Switch to an alternative approved manufacturer.
πŸ”Ή Stay updated through official ISA communications.
πŸ”Ή Report any suspected adverse drug reactions through appropriate channels.

Let's work together to ensure the highest standards of patient safety while the investigation is underway.
