உச்சநீதிமன்றத்தின் உத்தரவின்படி, அரசு பணியில் உள்ள மருத்துவர்களுக்கான 151 சூப்பர் ஸ்பெஷாலிட்டி இடங்கள் மத்திய ஒதுக்கீட்டிற்கு ஒப்படைக்கப்பட்டுள்ளன.

SDPGA-வின் தொடர் முயற்சிகளும், எங்கள் உறுப்பினர்களின் உறுதியான ஆதரவும் காரணமாக, அரசுடன் இணைந்து மேற்கொண்ட சட்டப் போராட்டத்தில் வெற்றி பெற்று, GO 462 மூலம் சூப்பர் ஸ்பெஷாலிட்டி படிப்புகளில் பணியிலிருப்போருக்கான ஒதுக்கீட்டை நீண்ட காலத்திற்கு முன்பே பாதுகாத்தோம். தற்போது, நிரப்பப்படாமல் உள்ள அரசு மருத்துவர்களுக்கான ஒதுக்கீட்டு இடங்களை மத்திய ஒதுக்கீட்டிற்கு ஒப்படைக்குமாறு உச்சநீதிமன்றம் மாநில அரசுக்கு உத்தரவிட்டுள்ளது. ஆண்டுதோறும் நமது அரசு மருத்துவர்களுக்கான ஒதுக்கீட்டு இடங்கள் காலியாக இருப்பது மிகுந்த வருத்தமளிக்கிறது.

இது மாநில அரசின் சட்டப் போராட்டத்தின் தோல்வி அல்ல. மாறாக, தமிழ்நாட்டின் அடுத்தடுத்த அரசுகள், சூப்பர் ஸ்பெஷாலிட்டி DM/MCh பயின்ற மருத்துவ பணியிடங்களை போதுமான அளவில் உருவாக்கத் தவறியதன் மூலம், அரசு மருத்துவர்களை அந்தப் படிப்புகளைத் தேர்வு செய்யாமல் இருக்கச் செய்துள்ளன.

இதுவே உண்மையான குறைபாடு.

ஒரு MD/MS மருத்துவர் உதவிப் பேராசிரியராக நியமிக்கப்பட்டு, பின்னர் இணைப் பேராசிரியர், பேராசிரியர், மருத்துவக் கல்வி இயக்குநரகத்தின் (DMER) டீன் போன்ற உயர்ந்த பதவிகளுக்கு பதவி உயர்வு பெற முடிகிறது.

ஆனால், மக்களுக்கு மேம்பட்ட மருத்துவ சேவையை வழங்கும் நோக்கில் கூடுதல் திறனைப் பெற DM/MCh படிப்புகளை முடித்த மருத்துவர்களுக்கு, பேராசிரியர், டீன் அல்லது மருத்துவக் கல்வி இயக்குநர் (DME) போன்ற பதவிகளை அடைவதற்கான வாய்ப்புகள் மிகவும் குறைவாக உள்ளன.

போதுமான உதவிப் பேராசிரியர் பணியிடங்கள், பதவி உயர்வு வாய்ப்புகள் மற்றும் உரிய ஊதியச் சலுகைகள் இருந்திருந்தால், அரசு மருத்துவர்கள் DM/MCh படிப்புகளில் அதிகமாக சேர்ந்திருப்பார்கள். அப்போது இந்த இடங்களை மத்திய ஒதுக்கீட்டிற்கு ஒப்படைக்க வேண்டிய நிலையே ஏற்பட்டிருக்காது.

இந்த முரண்பாட்டைச் சரிசெய்ய GO 354-ஐ மறுபரிசீலனை செய்ய வேண்டும் என்று SDPGA தொடர்ந்து வலியுறுத்தி வருகிறது. ஆனால், முந்தைய அரசுகள் இதை கவனிக்கத் தவறிவிட்டன.

35 ஆண்டுகளுக்கு முன்பு, சென்னை ஸ்டான்லி அரசு மருத்துவமனையில் தெற்காசியாவின் முதல் கல்லீரல் மாற்று அறுவைச் சிகிச்சையை வெற்றிகரமாக செய்த பெருமை தமிழ்நாட்டுக்கே இருந்தது. இன்று, சிறுநீரக மாற்று அறுவைச் சிகிச்சையைக்கூட சீராக மேற்கொள்வதில் சிரமங்களை எதிர்கொள்கிறோம்.

ஒரு காலத்தில் உறுப்பு மாற்று அறுவைச் சிகிச்சையில் இந்தியாவின் முன்னணி மாநிலமாக இருந்த தமிழ்நாடு, தற்போது மகாராஷ்டிராவிற்கு அடுத்த இரண்டாவது இடத்திற்கு தள்ளப்பட்டுள்ளது.

சுகாதாரத் துறை அதிகாரிகள், அரசு சுகாதார அமைப்பை திட்டமிட்டு படிபடியாக முறையாக பலவீனப்படுத்தி, தனியார் மருத்துவத் துறைக்கு சாதகமான சூழலை உருவாக்கி வருவதாகத் தெரிகிறது. கடந்த பத்து ஆண்டுகளில் தொடர்ச்சியாக ஆட்சி செய்த அரசுகள், தங்களது முன்னோடிகள் உருவாக்கிய உலகப் புகழ்பெற்ற தமிழ்நாடு சுகாதார அமைப்பின் பாரம்பரியத்தை முன்னெடுத்துச் செல்லத் தேவையான தொலைநோக்கு பார்வையை வெளிப்படுத்தவில்லை.

எனவே, சுகாதாரத் துறையில் மேலும் IAS அதிகாரிகளை நியமிப்பதை விட, சுகாதார சேவையை சமூகத்திற்கான செலவாக அல்ல, முதலீடாகக் கருதி, அதன் அடிப்படைப் பிரச்சினைகளுக்கு தீர்வு காணுமாறு தற்போதைய அரசை கேட்டுக்கொள்கிறோம்.

When we have been suffering from less manpower, poor pay, 24 hrs duty, delayed promotions, targets, review meetings etc now we are threatened with two new issues.

First one is redeployment of existing posts from one hospital to another hospital. Already we are deficient in numbers as per IPHS with respect to patient load. We need 35000 doctors but at present 18,000 and so we do two people's work. Now redeployment salt rubs our wounds. Officials consider we are all surplus and they translocate the posts from one hospital to another where it's demanded instead of creating new posts. That's why we demand Review of GO354. ஒவ்வொரு நாளும் நமது பணியிடம் நமக்கு தான் உள்ளதா என்பது கேள்விக்குறி.

Second, the government feels we are not performing well and not capable of doing administration. So they want to replace our Directors with IAS people and post DRO in Hospitals. So they ask us only to treat patients and teach students. Already they made us to beg them for salary. Now we need to toe them for attendance, leaves, increments, arrears, promotions etc. மொத்தத்தில் கொத்தடிமையாய் இருக்க சொல்கிறார்கள்.

Dark days ahead. We need to stand straight, stay united and raise our voice. We have shown our strength previously to obtain DACP salary and to retain the service quota. Now it's time to protect our dignified profession.

வெளிப்படைத்தன்மையுடன் நடைபெற்ற பணி மாறுதல் &பதவி உயர்வு கலந்தாய்வு

அரசு மருத்துவமனை வளாகங்களில் சமூக வலைதள காணொளிகள் (Reels) எடுப்பதற்கு தடை அறிவிப்புகளுக்கு, மாண்புமிகு மக்கள் நல்வாழ்வுத்துறை அமைச்சர் அவர்களுக்கும், சுகாதார செயலாளர் அவர்களுக்கும் நன்றியையும் *மருத்துவர் தின வாழ்த்துகளையும்* தெரிவித்துக் கொள்கிறோம்.

அரசாணை-113 மூலம் எங்களது தொடர் கோரிக்கையான சிறிய மருத்துவமனைக்களுக்கு தேவையான மருத்துவர் பணியிடங்களை உருவாக்கியமைக்கு நன்றி.
அதே வேளையில் பெரிய மருத்துவமனைகளில் இருந்து மாற்றம் செய்யப்பட்ட பணியிடங்களை மீண்டும் வழங்குவதோடு, அனைத்து அரசு மருத்துவமனைகளுக்கும் இந்திய பொது சுகாதார தர நிர்ணயத்தின் படி *IPHS* புதிய மருத்துவர் பணியிடங்களும், செவிலியர் உள்ளிட்ட துணை மருத்துவ பணியிடங்களும் உருவாக்க அரசாணை 113ஐ மறுஆய்வு செய்ய கோருகிறோம்.

2012க்கு பிறகு அரசு மருத்துவர்களின் எண்ணிக்கை, மக்கள் தொகைக்கு ஏற்ப உயர்த்தப்பட வில்லை. மேலும் அரசு மருத்துவர்களின் நீண்ட வருடங்களாக நிலுவையில் உள்ள கோரிக்கையான அரசாணை 354ஐ மறு சீராய்வு குறித்து அறிவிப்பினை வெளியிட,
இந்த மருத்துவர் தினத்தில் கோருகிறோம்.

அரசு மருத்துவர்களின் அயராத பங்களிப்புகள், தியாகங்கள் மற்றும் அர்ப்பணிப்பின் காரணமாக தேசத்தின் சுகாதார மாதிரி மாநிலமான தமிழ்நாட்டில், அரசு மருத்துவர்களின் மன அழுத்தம், மருத்துவர் இறப்புகளுக்கு, மருத்துவ சேவை குளறுபடிகளுக்கு காரணமான 24 /36 மணி நேர பணி முறையை ஒழித்திடவும் கோருகிறோம்.

SDPGA met Hon Health Minister at his office yesterday and presented our State EC meeting resolutions.

We thanked HM for not giving an extension to former Dean Dr Parthasarathy in the Director post of Kalaignar Centenary Speciality Hospital. We requested to Relieve all Retired Doctors functioning as Director TRANSTAN and Consultants in NHM, TNHSP and instead fill those posts appropriately from panel of Deans, Directors, Joint Directors from regular panel. He assured not to give extension to retired drs in future.

We thank the govt for relaxing one year station seniority and two years lock in ED and requested to continue the same till the promotions are done on time every year regularly. He assured us that there wouldn't be delayed promotions hereafter.

SDPGA has briefed about GO354, 245, 4D2, 293 related to our pay and promotional opportunities. We insisted about the long pending demand of Reviewing the GO that has resulted in monetary loss and stagnant promotions.

We explained in detail about the pros and cons of rationalisation GO113 in DMS. Because of the request of sdpga to increase the drs posts in smaller GH the GO is issued. But that is done by redistribution of posts including SCS/CCS through reduction of posts from larger GH. We demand the govt to address the issue by sanctioning extra posts wherever reduced before implementing the GO

We demanded to stop the redeployment to address the manpower shortage of Doctors. SDPGA stressed the immediate need of Staff nurses in all hospitals which is not only affecting the patient care but also burdens doctors during treatment. He accepted to address it immediately.

We explained about the inordinate delay of all promotions because of inadequate administrative posts and ministerial staff positions in DMER. He agreed to look into the structural issues by creating adequate Additional and Joint Director posts in DMER since the number of Medical Colleges have been increased

Requested to conduct Promotion Counseling CCS/SCS One-Time Fitment.

Regarding Service Quota DM/Mch lack of posts and Promotion for super speciality is reason for Service Postgraduate Doctors not taking DM/Mch so SDPGA requested post creation of AP, Associate professors in Super specialists dept in new medical colleges

We requested to address manpower shortages by creating Supernumerary posts for NSPGs in Medical Colleges and District Headquarter Hospitals.

We have clearly explained our pathetic condition of overburden work with increasing patient load, CMCHIS with less pay and delayed promotions. He said he would address our issues

We have requested to revoke the punishment transfer of Dr. Anton and to drop the charges against Dr. Perumal pillai. He said that would be done

🛡️Service Doctors and Postgraduates Association conducted the *State Executive Meeting in NLC Engineers Association Hall, Neyveli TS on 15.6.26 by SDPGA Cuddalore District* in a grand manner. District Office bearers from all over the State have attended and discussed various issues related to Patient care and Doctors Welfare.
The following resolutions were made:

1) The foremost issue is *severe shortage of manpower* that jeopardize both patient care and doctors' health.
Since Decade Recruitment of Doctors ,Nurses, Paramedicals have not been done properly and hence shortage exists in all Directorates.
*We request the Govt to pay immediate attention in filling up of the vacant posts in all cadres*.

2) Due to the high quality treatment and affordable care given to the public, Foot fall of patients to the Govt Hospitals is increasing day by day. But there has been no proportionate increase in Sanctioned posts of Doctors, Staff nurses, paramedics in last ten years. *We request the Govt to increase the Posts of Doctors, Nurses, Paramedicals, Security staffs etc as per IPHS norms*

3) Because of heavy work load with inadequate Manpower, Deputations and Diversions have become the Norms. Posts are being Redeployed based on minimum requirements of NMC Norms for MBBS,MD,MS,MDS,DM,Mch seats instead of patient care needs like Bed strength, Surgeries, Scans, Dialysis etc performed..*We request the Govt to stop Redeployment, Deputation and Diversion of Doctors*

4) Recently G.O-113 is issued to Rationalize the posts in secondary level DMS hospitals without taking IPHS norms into consideration and hence nearly 480 Doctors will be displaced from their original stations. Instead of creation of Posts for addressing Manpower Shortage they have gone for Redistribution of Posts from Larger hospitals to Smaller ones considering only Bedstrength as criteria with no consideration for patientcare, Specialist Services etc and this will affect the patient care only . *We request TN Govt to Sanction adequate Additional Posts of Doctors before implementing the Rationalisation process*.

5) Doctors and faculties are suffering from very delayed promotions at every level that cause monetary loss and make the system redundant. Every year the panel preparation of Directors, Deans, JDs, DHOs, faculties, CCS, SCS promotions are not done properly on time. *We request the govt to appoint one nodal officer at the level of Additional Director in each Directorate to look into timely Promotions and regular Transfers* so that the Department will be functional and effective.

5) *We request our present TN Govt to review G.O-354 issued in 2009 which is our very long pending important Demand of TN Govt drs.* The GO is very vital for TN Govt drs as it envisages adequate promotional posts and Time Bound salary structure. The G.O has the provision which mandates review every five years and the last review was done in 2012. Previous governments didn't do it inspite of repeated protests and losing our Great leader Dr.Lakshminarasimhan in the historic struggle. Every Govt Doctor has been affected with the loss of several Thousands of Rupees per month for the past Ten years because of the undue delay.

6) The most affected Dept is Super Speciality Doctors. Because of the want of adequate posts and delayed promotions many Drs are retiring as Assistant professors and lead to exodus from the service. It's a very pathetic condition for them to become subordinates with less remuneration for acquiring an additional degree and clinical expertise. *We request the govt to start super speciality service in all Districts in New Medical Colleges with adequate Promotional Posts and in Dist Hospitals attaching with near by medical colleges*

7) Though Tamilnadu has reduced Maternal Mortality rate to below national level, We can bring it much lower in par with developed countries. *We, SDPGA have been demanding from 2017 that all deliveries should happen in Cemonc centres with minimum 7 OG, 7 Pediatrics 7 Anesthetists and adequate paramedical staff to provide 24x7 service.*

8) Doctors are toiling during 24 hours duty which is affecting them and risking patients lives. Because of inadequate manpower, we are subjected to this inhuman duty pattern with a meagre 300 rupees allowance. No other dept or a cadre is witnessing such exploitation. If at all they do during they are given two days off or extra one day salary. *We request the Govt to adopt Shift duty pattern to safeguard the the Public as well as the professionals*.

9) Nowadays people *record videos on their mobile phones while visiting hospitals for treatment and post stray incidents as reels in social media to bring not only disrespect to doctors and nurses making them demoralised but also bad reputation to Govt hospitals. It creates nuisance to patients and violates their privacy. We request the govt to punish the reel makers through Hospital Protection Act and Cyber crime Acts as morethan 80% of Health manpower are Women and to sensitise the public through various forms.*

10) Primary Health Centres are the basic institutions to focus on public health and prevention is the motto to reduce the morbity and mortality. *We request the Govt to address the problems of the Medical officers to function effectively*
* by posting SCS/CCS as Block Medical Officers,
* by allocating enough funds on time,
* by reducing unnecessary paper works
* by posting round the clock security personnel

11) *We request to appoint the Medical officers as regular time scale posts in Urban PHCs* instead of contractual mode so that they can get the benefits of service quota in PG admissions, Eligible leaves, Pensions etc.

12) We demand to *Redesignate Diploma Doctors as Assistant professors* working in the concerned speciality on the DME side as per NMC norms.

13) The speciality Drs occupying NHM posts to be given teaching experience

14) Separate faculties to be posted for Allied Health Science courses and Nursing Colleges to be started in all Medical Colleges

16)Civil Medical List to be revised properly with the existing service conditions

17)Doctors are having work related personal grievances which are not addressed properly in time. *We need a Grievance register and Committee in every hospital* to address that swiftly and the action taken reports to be reviewed by the District and State Officers every month and by Directorate every Third month.

18)We welcome the online OP ticket system and request the Govt to revise the DMS side GH Timings from 8am to 12noon instead of 7.30am.

19)We need to recruit adequate Multi purpose health workers as there are increasing conflicts between sanitary workers and patients when they assist minor injuries and illnesses.

20)We request the govt to *abolish the Bond system* for doing Post graduation both service and non service in Tamilnadu. If at all bond is executed we demand To create supernumerary posts to accomodate for Non service PGs and remove the bond period till superannuation for service PGs.

21)We request the govt to provide *Voluntary Retirement Service* for govt Doctors, irrespective of speciality

22)We request the govt to post *Forensic medical specialists in all District Headquarters Hospitals*. To post Mortuary Assistants in all GH wherever postmortem done.

23)We demand the Govt not to construct or commission new buildings and Equipments without manpower. Already we are struggling a lot in vellore, sholinganallur, Ammapettai, Vriddhachalam, Krishnagiri etc. *No more new buildings or equipment without manpower creation*

24)Tamilnadu is the Pioneer and the leader in Organ donation and Transplant program. At present govt hospitals are lacking patronage to do it seamlessly. *We request the Govt to incentivise the specialists involved in the transplant program to make it successful*.

25) SDPGA demands the govt to allocate adequate funds to encourage research works

26) *Artificial intelligence to be incorporated* to improve the quality of health care services. We request the govt to bring apps to collect the particulars from drs in real-time so that timely leave sanction, arrears, joining relieving orders, promotions etc could be processed without delay.

27) *TNMSC should take up the responsibility to procure all Drugs, Consumables, instruments, equipments and to be accountable for the quality of such products.* We are facing departmental actions and threats from the public. Example breaking of epidural catheter in Tiruvallur medical College, Adulterated Dexamethasone injection in Trichy Medical College etc.

28) SDPGA suggests keeping a sample vial/ampule of every medicine used, till the usage of last vial/ampule of medicine of same batch, as followed in food samples from hospital kitchen till next day for further analysis if untoward reactions happen to patients.

29)State EC resolved to form a federation involving health care providers to strengthen our demands of manpower, work atmosphere, security etc
