Luxation, tooth & bone fracture.
3 months of splinting + rehab (without implants)

This gentleman had a fall. He had protruding maxillary anteriors, which are anyway prone to such injuries.

On clinical examination 11, 21, and 22 were damaged to verifying degrees. I took a CBCT and IOPAs to access the damage (if any) on the roots & bone. What the CBCT revealed was that while the roots were fine, the cortical plate was fractured and 21 had luxated

After anesthetizing the patient, I gently moved tooth 21 within the socket and removed the palatal fractured fragment on 22. I then proceeded to re-bond a broken fragment on 11 (the patient had brought it along) and splint all 3 teeth with composite.

I recalled the patient after 24 hrs. as I couldn’t really control the bleeding on day one to effectively splint with composite. I kept seeing him regularly for 2 months. As the area healed, he lost a lot of hard/soft tissue on the labial side on 22 which was unfortunate but we were absolutely blessed with his ultralow lip line!

We proceed with root canal treatments 21 and 22. A month later I removed the splints and started the prosthetic rehabilitation. Which included 2 splinted crowns on 21 and 22 and a veneer on 11.

This lovely 51-year-old woman, residing in London, had been planning to improve her teeth for some time. Even before meeting me, she was aware of her options (she had consultations back home). After extensive discussions, we agreed on the following aesthetic goals: to align, brighten, and enhance the volume of her smile. Initially, we planned to only work on her upper teeth, but after completing them, she requested to also have her lower teeth done.

I performed the entire procedure without the use of any prep guides or mock-ups. Although this may seem counterintuitive, I believe that with an understanding of the patient's expectations and my own abilities, such tools are not always necessary. In fact, a test drive is not possible in such “reductive” cases.

Once the veneers were ready, we temporarily placed them using a trial paste to take photos and videos and obtain informed consent from the patient regarding the final appearance before bonding.

For academic and learning purposes, I combined preoperative, tooth preparation, and postoperative scans, and created sections of the front four teeth to evaluate the amount of tooth reduction and veneer thickness. These sections show that more aggressive preparation was done on the lateral incisors, while almost no labial reduction was required on the right central incisor, which was positioned lingually.

The making of a smile with Emax veneers

This case involved a comprehensive dental treatment aimed at enhancing the overall appearance of the patient's smile. By opting for this holistic approach, we could exercise complete control over the color, shape, size, and proportions of each tooth. Since there was no need to match the veneers to any natural teeth, we used monolithic Emax veneers instead of layering. This option is more efficient, less cumbersome, and faster to make. Additionally, this approach significantly reduced the risk of fractures in the incisal areas.
In addition to aesthetic improvement, we also identified demineralization zones in almost all interproximal areas in the upper anterior regions. As a result, we opted for the slice tooth preparation approach, which involved opening the contact points. This technique allowed the veneers to start more lingually and create a hygienic ceramic-to-ceramic contact point. We also employed guided tooth preparation techniques, which allowed for adequate yet minimal reduction, resulting in faster tooth preparations.
Conversely, in the lower arch, we were able to take a more conservative approach to the preparation design due to the correct alignment of teeth and the absence of demineralization.
