Sometimes Maxillary seven are hiding some anatomies which is revealed only by using a microscope . presenting a case of performing an endodontic procedure in 17, where MB & DB canals are too close to explore with the naked eye and give the impression of having only one buccal canal. But when I saw under a microscope, I got to see both the buccal canals, which are located just 1 mm apart . i prepare them by using MG3 Blue files from perfect endo and obturated them using a single cone with bioceramic sealer . NAOCL 5.25 % & 17%EDTA IS USED as irrigants.

A 35-year-old patient presented with a history of trauma following a fall. Clinical examination revealed an Ellis Class III fracture involving tooth 21 with pulpal exposure. Vitality testing was positive. Root canal treatment followed by direct composite restoration was planned.
First visit: Endodontic treatment cleaning and shaping was completed using MG3 Blue files by perfect endo and irrigation using 5.25% sodium hypochlorite, followed by obturation using a bioceramic sealer and the cold lateral compaction technique.
Second visit: Definitive coronal rehabilitation was completed with a direct composite build-up, restoring the tooth's esthetics and function using tokuyama Asteria composites .

Sharing a rare case of calcified palatal canal along with huge pulp stone removal from maxillary seven in a patient aged 35 .
Case completed in 3 visits where 2 visits are gone just to remove calcifications and preperation of canals .
Obturation was done in third visit

There was deep class 5 caries in seven leads to pulp exposure .

DTE ED3D ultrasonic tip from woodpecker was used to remove pulp stone from chamber and palatal canal .

Cleaning and shaping was done by MG3 blue files from perfect endo .

5.25 % NAOCL ,normal saline & 17% EDTA Soln was used as an irrigation along with PUA by Ultra x

Single cone Obturation was done by bioceramic sealer from dentessa .

I am the only one who feels that finding MB2 in maxillary seven is common now a days or you are also getting it ??
I get it in every 3 rd case now !

Patient came to us with severe pain in her upper back tooth since 1 week .

O/E 17 was tender on percussion and an IOPA was taken . There was a crown there on 17 without doing a root canal treatment .

Pt don’t want to get this crown replaced for now so we decided to perform Root canal treatment through crown after taking an informed consent from the patient .

I made an access through crown and found that pulp chamber is calcified . Pulp stone was removed by ultrasonics with the help of magnification and illumination .

I found MB2 also in this and did thorough cleaning and shaping with MG3 blue files from perfect endo along with copious irrigation with 5.25% NAOCL ,saline and 17% EDTA solution was used at last to remove the smear layer.

Single cone obturation was done using Atyanta Bioceramic sealer .

Access cavity was close by direct composite restoration .

This series of IOPAs highlights one of the most common challenges in endodontic treatment: a severely curved distal root that tests both technique and patience. Such cases push the limits of clinical precision, where even minor deviations in technique can result in ledging, instrument separation or worse - procedural failure.
Successful management hinges on several key principles:
Meticulous glide path creation with pre-curved stainless steel K-file preferably #8 and #10
Regular irrigation with 5.25% NaOCl, 17% EDTA soln. and activation by UltraX to ensure debridement in curved apical segments
All the canals are prepared till 30 04 MG-3 Blue from PerfectEndo

Respecting canal anatomy is key along with patience and good technique.

Nowadays I always give buccal infiltration followed by intraligamentary injection whenever I have to perform a root canal treatment of mandibular molars . It is 100% effective without any miss . Sharing a small video demonstrating the same .
