This series of IOPAs highlights one of the most common challenges in endodontic treatment: a severely curved distal root that tests both technique and patience. Such cases push the limits of clinical precision, where even minor deviations in technique can result in ledging, instrument separation or worse - procedural failure.
Successful management hinges on several key principles:
Meticulous glide path creation with pre-curved stainless steel K-file preferably #8 and #10
Regular irrigation with 5.25% NaOCl, 17% EDTA soln. and activation by UltraX to ensure debridement in curved apical segments
All the canals are prepared till 30 04 MG-3 Blue from PerfectEndo

Respecting canal anatomy is key along with patience and good technique.

Nowadays I always give buccal infiltration followed by intraligamentary injection whenever I have to perform a root canal treatment of mandibular molars . It is 100% effective without any miss . Sharing a small video demonstrating the same .

Sharing an interesting referral case of root canal treatment of teeth 26 & 27 where previous dentist had already started root canal for these same teeth because patient was having a sever pain in her left upper jaw .
Now the interesting thing when i checked patient clinically ,i could not found anything which is causing that sever pain to patient . i can see the access was good and canals were shaped quiet well . only one thing i found was the inadequate debridement of pulp ,so i prepared all the canals well along with a lot of irrigation and activation of 5.25 % NAOCL because that might be a common reason for pain .

what the most interesting part of this case was symptoms of pain of the patient forced me to think about TRIGEMINAL NEURALGIA .
So when i called patient the alternate day she started complaining of pain in maxillary left canine and told me to extract it as pain was horrible and she was crying, but canine was absolutely normal in clinical findings . This clears my confusion and we started medication for Trigeminal neuralgia .
Guess what???? patients pain was reduced to 60% after only 3 days of medication for TGN.

I obturated the 26,27 in second visit with WVC followed by direct composite build up .

SO CONCLUSION OF SHARING THIS CASE IS BEING A DENTIST WE SHOULD NOT JUMP TO THE TREATMENT IMMEDIATELY WHENEVER PATIENT COMES TO US WITH PAIN . WE ALL SHOULD LISTEN TO THE SYMPTOMS OF PATIENTS CAREFULLY AND RE CHECK THEM WITH OUR CLINICAL FINDINGS . IF ALL CLINICAL FINDINGS ARE ABSOLUTELY NORMAL THEN THINLK OUT OF THE BOX SOMETIMES .

case completed in 2 visits .
MB1 & MB2 are having separate apical exits.
5.25 % hypo ,saline & 17% EDTA was used as irrigants
Activation of hypo was done by PUA
Palatal canal system is beautiful ❀
Obturation was done by WVC followed by direct composite buildup.

Sharing a referral case of file retrieval from ML canal of 36.
Reason behind this file separation was inadequate access AND very thin root canal system .
Adequate access is the ultimate key to find all the canals and prevent these kind of mishaps...
We should always avoid making ninja access and very small access .
