MHN- antipsychotic agents
(This topic is from 9th question of NCLEX preparation posted earlier.)

👉🏻Dystonia- involves grimacing, torticollis, intermittent muscle spasm.

👉🏻Pseudoparkinsonism- resembles true parkinsonism (tremor, mask like facies, drooling, restlessness, shuffling stooped gait, rigidity)

👉🏻Akathisia- motor agitation (restless legs, "jitters", nervous energy)

👉🏻Akinesia- fatigue, weakness, painful muscles, lack of energy

👉🏻Tardive dyskinesia- late appearing after prolonged use of antipsychotics. Not related to dopamine anticholinergic imbalance. Characterized by involuntary movt. of face, jaw and tongue; lip smacking, grinding of teeth, rolling/protrusion of tongue, tics, diaphragmatic movts. that may impair breathing

Pharmacology- Routes of drug administration
(PART-2)

👉🏻Some imp. points:-
A) Adhesive units- Adhesive patches of diff. sizes and shapes made to suit the area of application. The drug is held in reservoir between outer layer and a porous membrane. The drug slowly diffuses through the membrane and percutaneous absorption takes place. Highly potent and short acting drugs are suitable for use in such system.
Sites of application are:
1) Chest
2) Abdomen
3) Upper arm
4) Back/mastoid region

B) Inunction- Drug is rubbed in to the skin and it gets absorbed to produce systemic effects.

C) Iontophoresis- Galvanic current is used for bringing about pe*******on of lipid insoluble drugs into the deeper tissues where it's action is required.

D) Ocusert- are thin elliptical units that contain the drug in a reservoir which slowly releases the drug through a membrane by diffusion at a steady rate. Eg., pilocarpine ocusert used in glaucoma.

E) Progestasert- Inserted into the uterus where it delivers progesterone constantly for 1 year.

F) Osmotic pumps- small tablet shaped units containing the drug and an osmotic substance in 2 diff. chambers. The tablet is coated with a semipermeable membrane in which a minute laser-drilled hole is made.

G) Computerized miniature pumps- These are programmed to release drugs at a definite rate either continuously as in case of insulin.

H) Monoclonal antibodies- Antibodies against the tumor specific antigens and used to deliver anticancer drugs to specific tumor cells.

I) Liposomes- are phospholipids suspended in aqueous vehicles to form minute vesicles.

Pharmacology - Routes of drug administration
(PART-1)

👉🏻Some imp. points:-
A) Dermojet- A high velocity jet of drug solution is projected from a fine or***ce using a gun. The sol. gets deposited in the SC tissue from where it is absorbed. This method is painless and suitable for vaccines.

B) Pellet implantations- Small pellets packed with drugs are implanted subcutaneously. The drug is slowly released for weeks or months to provide constant blood levels. Eg., testosterone.

C) Sialistic implants- The drug is packed in sialistic tubes and implanted subcutaneously. The drug gets absorbed over months to provide constant blood levels. Eg., hormones and contraceptives. THE EMPTY NONBIODEGRADABLE IMPLANT HAS TO BE REMOVED.

D) A bolus- Where an initial loarge dose is given, eg. heparin. The drug is dissolved in a suitable amt. of vehicle and injected slowly.

E) Slowly- Over 15-20 min. eg., aminophylline.

F) Slow infusion- When constant plasma concentrations are required (oxytocin in labor) or when large volumes have to be given (dextrose, saline). Rate of infusion depends on patient's condition.

Route and preferred size:
1) SC - 25G
2) IM, IV - 22-24G
3) Intradermal - 26G
