We often judge people by what we can see.

The smile.
The silence.
The anger.
The distance.

But mental health nursing taught me that every behavior has a story, even if we don't know it.

Not every person with depression looks sad.
Not every person with anxiety looks nervous.
Not every struggle is visible.

The biggest lesson I learned wasn't about diagnoses—it was about compassion.

The next time someone's behavior doesn't make sense to you, try replacing judgment with curiosity. You may never know the battle they're fighting.

Most students get confused when they see K⁺ = 5.8 mEq/L in DKA

Yes, serum potassium is high—but total body potassium is actually low

Why?
• No insulin → K⁺ shifts out of cells.
• High glucose → osmotic diuresis → K⁺ is lost in urine.

Insulin is still the treatment because it stops ketone production and corrects DKA. As insulin is started, K⁺ moves back into cells, so potassium can drop rapidly.

**NCLEX tip:** If **K⁺ < 3.3 mEq/L**, replace potassium before starting insulin.
