Retrieving a broken implant screw is a technically challenging but common procedure in dentistry. The approach depends on the location of the broken screw, the type of implant system, and whether the screw fragment is mobile or embedded. Here's a general overview of techniques used for broken screw retrieval:
Dental Implant Broken Screw Retrieval Techniques

Step 1: Assess the Situation
• Radiograph: Confirm the presence and position of the broken fragment.
• Access: Remove the crown or prosthesis to expose the implant head.
Step 2: Choose the Appropriate Retrieval Technique
1. Manual Techniques
• Explorer or Probe: Try to gently rotate the fragment out if it's loose.
• Ultrasonic Scaler: Vibrations can sometimes loosen the fragment.
• Sharp Instrument: A small endodontic file or explorer can engage the groove of the screw to back it out counter clockwise.
2. Using a Screw Retrieval Kit
Many implant systems (e.g., Nobel Biocare, Straumann, Zimmer) offer screw retrieval kits with:
• Reverse-threaded drill bits
• Extraction tools
• Trephines or hollow burs to engage the screw and spin it out
Note: Be sure to use the manufacturer-specific retrieval kit if available.
3. Create a Slot
If the top of the broken screw is visible:
• Use a high-speed handpiece with a fine bur to create a slot
• Use a screwdriver or flat-head tool to unscrew it.
4. Drill-Out Technique
As a last resort:
• Carefully drill out the fragment with a carbide or reverse-taper bur
• Preserve implant threads if planning to reuse the implant
• Use copious irrigation to prevent overheating
A SIMPLE YET EFFECTIVE TECHNIQUE RECENTLY USED WAS USE OF AN EAR BUD TO REMOVE THE SCREW WHEN IT BREAKS IN THE IMPLANT HEAD.
• ENGAGE THE EAR BUD TO THE SCREW
• HOLD TIGHT AND ROTATE ANTI CLOCKWISE.
