ট্রায়াজ (Triage) একটা মেডিকেল কোড। খুব নিষ্ঠুর এবং ভয়াবহ একটা কোড। যেটা উপায় না থাকলে বৃহত্তর স্বার্থে প্রয়োগ করতেই হয়। ব‌ইয়ে এর কথা পড়েছি। যুদ্ধক্ষেত্রে আর্মি ডাক্তাররা এটা apply করেন।

ধরুন ভারত পাকিস্তান যুদ্ধ লাগলো - কথার কথা বলছি বোঝানোর জন‍্য। এবার তাতে - 50 জন ভারতীয় সেনা আহত হলেন। কিন্তু যুদ্ধক্ষেত্রে ডাক্তারি যন্ত্রপাতি তো খুব বেশী পরিমাণে থাকেনা। অস্হায়ী পরিকাঠামো।দেখা গেল - war field এ পরিকাঠামো আছে 20 জনকে চিকিৎসা দেওয়ার মত। এবার ওখানে ডাক্তার যিনি দায়িত্বে আছেন, তার কাজ ট্রায়াজ করা। উনি নির্ধারণ করবেন। কি নির্ধারণ করবেন? কার injury severity কতটা। কাকে বাঁচানো সম্ভব এবং রাষ্ট্রের বৃহত্তর স্বার্থে সে কতটা কাজে লাগবে। মানে খুব সোজা ভাষায় বললে - যার তুলনামূলক অল্প আঘাত, এবং চিকিৎসায় সাড়া দিয়ে সুস্থ হবে এবং আবার যুদ্ধে ফিরতে পারবে (রাষ্ট্রের বৃহত্তর স্বার্থ কথাটা important) তারাই শুধুমাত্র চিকিৎসা পাবে। বাকী 30 জন আহত চিকিৎসা পাবেনা পরিকাঠামোর অভাবে। With due respect for all soldiers, দেশের দশের, সমাজের বৃহত্তর স্বার্থে রাষ্ট্র দেখবে - "ওই 30 জন, যারা মরণাপন্ন অবস্থায়, বাঁচানো সম্ভব না, এত severely injured, ওরা দেশের মানুষকে বাঁচানোর কাজে যুদ্ধে ফিরতেও পারবেনা। আমার limited medical resource ওদের জন‍্য খরচ করা মানে এই vital moment এ সেটা নষ্ট করা।"

এবার বাস্তবে ফিরি।

পরিস্থিতি যেদিকে এগোচ্ছে, ট্রায়াজ হয়তো apply করা ছাড়া উপায় থাকবেনা। করোনার bed crisis, oxygen crisis, ওষুধের crisis এমন জায়গায় যাচ্ছে।

একটু সোজা ভাষায় বুঝিয়ে বলি। ধরুন একটা পরিবারে মা বাবা আর তাদের ছেলে মেয়ে। ধরুন মেয়েটি আমার বয়সি। তারা সবাই করোনা আক্রান্ত হলো।তাহলে ট্রায়াজ apply হলে কি দাড়াবে?

ভর্তি নিয়ে ট্রিটমেন্ট দেওয়ার মত bed, infrastructure, পরিকাঠামো, oxygen ইত‍্যাদির অভাব। বাবার বয়স হয়েছে এখন, রিটায়ার্ড। মানে দেশের বৃহত্তর কাজে, সমাজের কাজে সে আসছেনা। মায়ের বয়স ও বেশী। এঁরা elderly population. হয়তো করোনা হলে এঁদের severity এতটাই বেশী হবে, যে এদের ভর্তি করে চিকিৎসা দিলেও তার survival chance কম। মা, বাবা ভর্তির বেড পাবেনা। Elderly population will be excluded first following triage. নতুন প্রজন্ম, সামনে ভবিষ‍্যৎ, এবং তারা দেশের কাজে আসবে। এবং ছোটদের severity কম হবে, immunity বেশী। সুতরাং যেটুকু resource আছে, যেটুকু oxygen আছে - এদের দিলে হয়তো কাজে লাগবে। যে কটা বেড আছে - এদের ভর্তি করলে সেটা আখেরে বৃহত্তর জনস্বার্থে ফলপ্রসূ হবে।

তাহলে সারাংশ কি দাড়ালো? চোখের সামনে বাচ্চা ছেলেটি, বা মেয়েটি দেখবে - আমরা ভর্তি হলাম, চিকিৎসা পেলাম। কিন্তু বাবা মা চিকিৎসা, পরিকাঠামোর অভাবে এবং ট্রায়াজ applied হয়ে ভর্তি হতে পারলেন না। চিকিৎসা পেলেন না। ট্রায়াজ এর ভয়াবহতা এতটা বেশী।

Italy তে করোনা চিকিৎসায় ট্রায়াজ applied হয়েছিল। ইটালির president এর কান্নার ভিডিও টা মনে আছে? ভারতের ও হয়তো ট্রায়াজ apply করা ছাড়া কোন উপায় থাকবেনা ।
তবুও আপনি-

1. মাস্ক ব্যাবহার করবেন না।
2. দূরত্ব বজায় রাখবেন না।
3. জমায়েতে অংশগ্রহণ করবেন।
4. যারা মাস্ক পড়বেন তাদের টিটকারি করবেন।
5. হাত স্যানিটাইজ করবেন না।
6. বিনা প্রয়োজনে বাজারে যাবেন।
