What is RIPA test ?
= :Introduction
Ristocetin is an antibiotic but was found to cause thrombocytopaenia and so removed from the market. Ristocetin induces binding of von Willebrand Factor [VWF] to the GpIb complex by an alteration of the electrostatic forces between GpIb and VWF in the micro-circulation.
Physiologically, it is the high shear rates in the capillary circulation that leads to the unfolding and then binding of VWF to platelets rather than a Ristocetin-like molecule.

🔬 Left Shift in Hematology
Definition
• A left shift means an increase in immature neutrophils in peripheral blood.
• Normally, circulating neutrophils are mostly segmented (mature) forms.
• When the demand for neutrophils is high (infection, inflammation, marrow stimulation), the bone marrow releases earlier precursors (bands, metamyelocytes, and sometimes myelocytes).

✅ Normal Distribution of Neutrophils
• Segmented neutrophils (segs): 40–70% of circulating WBCs
• Bands: 0–5% of circulating WBCs
• Metamyelocytes & myelocytes: not normally seen in peripheral blood (except in severe stress, marrow disease, or myeloproliferative states)

🚨 Left Shift Criteria
• Bands > 10% of total WBCs
OR
• Immature granulocytes (bands + metamyelocytes + myelocytes) > 1,000/µL absolute count

🔎 Causes of Left Shift
1. Infections (bacterial sepsis, pneumonia, abscess)
2. Inflammation (tissue injury, burns, myocardial infarction)
3. Bone marrow stimulation (growth factor therapy, recovery after chemotherapy)
4. Myeloproliferative neoplasms (CML, leukemoid reaction)

📌 Clinical Significance
• A mild left shift (↑ bands only) → often bacterial infection.
• A marked left shift (bands + metamyelocytes + myelocytes in blood) → severe infection, sepsis, or bone marrow stress.
• Presence of toxic changes (toxic granulation, Döhle bodies, cytoplasmic vacuoles) with a left shift further supports acute bacterial infection.

⚖️ Key Point on Bands
• Normal: ≤ 5% of neutrophils
• Left shift: > 10% bands, or bands present along with earlier precursors
• Example: WBC 20,000/µL with 20% bands = absolute band count 4,000/µL → significant left shift
