PSA 15 — এখন কী করবেন?

PSA 15 হলে প্রথমেই কী করতে হবে?

প্রথমেই একজন Uro-Oncologist-এর পরামর্শ নিন।

তিনি আপনার সম্পূর্ণ medical history নেবেন এবং আপনাকে পরীক্ষা করবেন।

প্রথমেই দেখতে হবে, কোনও infection আছে কি না।

🔹 Urine routine examination
🔹 প্রয়োজনে Urine culture

এরপর Digital Re**al Examination (DRE)-এর মাধ্যমে prostate পরীক্ষা করা হয়—কোনও nodule আছে কি না এবং prostate-এর consistency কেমন, তা বোঝার জন্য।

কিছু ক্ষেত্রে PSA repeat test করারও প্রয়োজন হতে পারে।

তবে PSA যদি উল্লেখযোগ্যভাবে বেশি থাকে, তখন Multiparametric MRI (mpMRI) of prostate করার প্রয়োজন হতে পারে।

mpMRI কীভাবে সাহায্য করে?

এতে prostate-এর মধ্যে কোনও suspicious nodule বা lesion আছে কি না, থাকলে তার বৈশিষ্ট্য কী—তা বোঝা যায় এবং PI-RADS scoring-এর মাধ্যমে cancer-এর সম্ভাবনা নির্ণয়ে সাহায্য করে।

MRI ও অন্যান্য clinical findings-এর ভিত্তিতে প্রয়োজন হলে prostate biopsy করতে হতে পারে।

👉 মনে রাখবেন—PSA 15 মানেই prostate cancer নয়।
কিন্তু এটিকে অবহেলা করাও উচিত নয়।

সময়মতো সঠিক evaluation করলে prostate cancer থাকলেও early stage-এ শনাক্ত করে সম্পূর্ণ নিরাময়ের সুযোগ অনেক বেশি থাকে।

তাই শুধু PSA report হাতে নিয়ে বসে না থেকে সঠিক সময়ে একজন Uro-Oncologist-এর পরামর্শ নিন।

Young, Fit Patient With Localized Prostate Cancer: Is Robotic Surgery the Right Choice?

A diagnosis of prostate cancer can be frightening—especially when the patient is relatively young, fit and otherwise healthy.

But not every prostate cancer needs the same treatment.

👨‍⚕️ When can robotic prostate surgery be particularly attractive?

Consider a 58-year-old fit man whose prostate cancer is detected early and appears confined to the prostate.

For such a patient, Robotic Radical Prostatectomy (RARP) may be an excellent treatment option, particularly when:

• He has a long life expectancy—often 10–15 years or more
• The cancer is localized, especially intermediate-risk disease
• He has good urinary control and sexual function before treatment
• Nerve-sparing surgery is technically possible
• He prefers surgery over radiotherapy
• He wants the prostate removed and a detailed pathological examination of the entire specimen

One important advantage of surgery is that we obtain the complete pathological picture—the exact extent and grade of the cancer, surgical margins and lymph-node status when nodes are removed.

And if PSA rises after surgery, radiotherapy can still be considered as salvage treatment in appropriate patients.

☢️ But what about radiotherapy?

Radiotherapy is also an excellent curative treatment and may be particularly attractive for some patients—such as older or less fit men, patients with significant pre-existing urinary problems, or those with high-risk/locally advanced disease where radiation is combined with hormonal treatment.

So, which is better?

There is no single treatment that is best for every patient.

For a young, healthy man with cancer confined to the prostate, robotic prostatectomy can be a very attractive option because it offers definitive removal of the prostate, detailed pathological staging and the possibility of preserving urinary and sexual function when appropriately selected.

The decision should always be individualized after discussing cancer control, urinary function, sexual function, bowel effects, age, life expectancy and the patient’s own priorities.

Early diagnosis gives us more choices. The right treatment is the one that is right for the individual patient.

🩺😂 FUN FACT IN UROLOGY –5

—-The Unexpected Beginning of Nephrectomy—-

The history of nephrectomy is not at all straightforward.

The first successful planned nephrectomy was not performed for a kidney tumour or even for a primary kidney disease.

It was performed to cure a woman from continuous leakage of urine through the va**na.

In 1869, German surgeon Dr. Gustav Simon treated a patient who had developed a ureterova**nal fistula following a gynaecological operation.

The situation was devastating.

Continuous leakage of urine caused severe skin irritation and excoriation, discomfort and an unbearable smell. In the social circumstances of that era, a woman with constant urinary leakage could even become socially isolated or an outcast.

Simon identified the problem:
urine from the kidney was passing through the ureter and leaking into the va**na.

But in 1869, reconstructing or reimplanting the damaged ureter was not a realistic option.

So Simon asked a remarkable question:

“If I cannot repair the ureter, can I eliminate the source of the urine?”

His answer was radical:

Remove the affected kidney.

He performed the nephrectomy—and remarkably, the patient survived.

This became the first successful planned nephrectomy in a living human, establishing that a person could survive with a single kidney.

Today, of course, a ureterova**nal fistula is generally treated by ureteric reimplantation or other reconstructive techniques, preserving the kidney whenever possible.

But Simon’s operation laid an important foundation for the renal surgery we know today—including nephrectomy for kidney tumours and other serious renal diseases.

And perhaps the greatest lesson from this story is:

Today’s advanced treatments are often the result of years of observation, experimentation, courage—and the willingness to think beyond the limitations of one’s time.
