Mostly 'Wh' questions are framed using the following format (Present Indefinite Tense)
Wh word + "do" verb + subject + main verb + rest (other words)
1) I live in Kolkata. (Where)
Wh Question:- Where do you live?
2) He walks slowly. ( How)
Wh Question:- How does he walk ?
3) I complete my breakfast at 8 am. (When)
Wh Question:- When do you complete your breakfast ?
4) She wants the new book. (What)
Wh Question:- What does she want ?
5) I want this pen because I like this one so much. (Why)
Wh Question:- Why do you want this pen.
