Tenecteplase (TNK Injection)
What is TNK?
Tenecteplase (TNK) is a clot-busting (thrombolytic) medicine.
It dissolves blood clots and restores blood flow.
Main Indications
Acute ST-elevation myocardial infarction (Heart attack)
Selected patients with Acute ischemic stroke (where recommended by stroke protocols and specialists)
Dose
STEMI: Single IV bolus, weight-based dosing.
Acute ischemic stroke: 0.25 mg/kg IV bolus (maximum 25 mg) according to current stroke protocols.
Administration
Reconstitute with the supplied sterile water according to the manufacturer's instructions.
Give as a single IV bolus over about 5–10 seconds.
Flush the IV line with normal saline before and after administration.
Do not mix with dextrose-containing solutions.
Contraindications
Active internal bleeding
Previous intracranial hemorrhage
Suspected aortic dissection
Severe uncontrolled hypertension
Recent major surgery or significant head trauma
Possible Complications
Major bleeding
Intracranial hemorrhage
Allergic reactions (rare)
Reperfusion arrhythmias (after treatment for STEMI)
Nursing Responsibilities
Confirm eligibility and contraindications before giving TNK.
Record the exact symptom onset time.
Monitor vital signs and neurological status.
Watch closely for signs of bleeding.
Avoid unnecessary intramuscular injections and invasive procedures after thrombolysis.
Important: TNK should only be administered by trained healthcare professionals in an emergency setting after confirming that the patient meets eligibility criteria.
