The root exposure due to faulty brushing or may position of the teeth is common. The coronally advanced flap with sub epithelial connective tissue is the most common approach to treat this defect. The use of CTG is subjective to the biotype and extent of the defect. A Thin biotype necessitates a CAF with CTG where as a Thick biotype may not need a CTG along with CAF.
The results achieved with this technique are stable and for how long? .. this case presented is now 14 years and counting...gosh am i practicing for this long or am i getting old...

Hi,
This case is of a 54 year female patient reporting to us for replacement of her missing teeth. She had infection and sinuses with a few of her teeth, mobility with a few and few restored teeth.
The treatment plan was to rehabilitate with implant in the maxilla and mandible. At the initial part of the treatment was pre-pandemic and later half during the pandemic... we saw her last week and that how she is...

Long case:
32 year female patient: reported for replacement of her missing teeth in 2013. She had lost the teeth due to periodontal disease.
Most of the teeth were mobile and the patient was told that she will be edentulous. It took the patient 3 months to get to the fact that she will have no teeth. eventually all her teeth were taken off and a denture was fabricated.
Being a teacher she needed fixed teeth. the maxillary denture was stable and she decided to to get the lower jaw fixed for the time being.
The clinical situation mandated an all on 4 prosthesis. the patient was then rehabilitated with 4 implants in the mandible and a metal-acrylic hybrid prosthesis was delivered in 2013.
By 2019, her maxillary denture started being not so stable and she decided to undergo a fully fixed prosthesis. she had saved money for the upper.
the bone available in the maxilla for an all on 4 kind of prosthesis limited the distal implant to the canine area. so it was decided to do a bilateral sinus graft and then get the implants in the bone.
The implants were placed in the sinus graft and as nature would have it the implant in the left sinus graft failed. I then did not want to do one more implant in the failed sinus. convinced the patient to get a zygoma implant done and then the patient agreed. the patient was restored with a screw retained PFM in the maxilla opposing a acrylic hybrid in the mandible.
