Plunging Ranula Explained 🩺💧 | The Neck Swelling That Starts Under the Tongue
A plunging ranula is not just a simple oral cyst. It occurs when mucus from the sublingual salivary gland escapes through the mylohyoid muscle and extends into the neck, producing a soft cystic swelling. Unlike a simple ranula confined to the floor of the mouth, plunging ranula presents as a neck mass and can mimic other cystic lesions.
🔍 Key points:
• Origin → Sublingual gland
• Cause → Mucus extravasation
• Presentation → Neck swelling ± oral swelling
• Imaging → CT/MRI shows cystic lesion crossing mylohyoid
• Treatment → Excision of sublingual gland with drainage
A must-know surgical and radiology concept for medical students and residents.

Ugly Duckling Sign 🦆 | Spot Melanoma Early Before It Spreads
One mole can kill… but only if you ignore the odd one out.
The Ugly Duckling Sign is a powerful clinical clue in dermatology and surgery. Instead of analyzing every mole individually, compare them with each other. Most moles follow a pattern, similar size, color, and shape.
But when one mole stands out as different, that’s your red flag 🚨
This odd one out could be melanoma, a potentially deadly skin cancer if missed early. Identifying it early can literally save a life.
Look for a mole that is
Darker than others
Larger than surrounding moles
Irregular in shape
Changing faster than the rest
Clinical takeaway
Don’t compare with textbooks. Compare with the patient’s own moles.
