Why problem of healing?
-No cambium layer in the intracapsular area, so no peripheral callus. Healing is only by endosteal callus.
- Synovial fluid lyses blood clot at the fracture site & there by destroys another mode of secondary healing.
- Displaced fracture leads to avascularity.
Fracture of the Hip joint.
Plain x ray of the hip, AP & lateral views help to make a diagnosis.
Fracture of neck femur could be intracapsular or extracapsular.
Intracapsular neck femur is known as an orthopaedic enigma.
Etiology :-
It is common in older patients with osteoporosis or osteomalacia & in them usually it is fracture through a pathological bone.
Complications of femoral neck fracture.
- Thromboembolism
- Non union
- Avascular necrosis

It a endurance disorder with widespread musculoskeletal Pain all over the body. .
incidence after osteoarthritis this is the most common rehumatological disorder. The overall incidence is 2.5% with women suffering 8-10 times more than men.

Causative factors:-
- In 5 - 10% it could be hereditary.
- In majority, any condition that condition that lowers the endurance of the muscle can trigger this condition. Notorious among them are sleep disorders loss of stage IV, delta wave sleep, trauma, connective tissue disorders, infection etc.

Clinical features :-
Pain with increased activity, stress or poor sleep
fatigue ,stiffness ,arthralgia
headache ,chestpain ,abdominal pain
📚📚
Diagnosis :-
-Widespread pain for at least 3 month in four limbs .
- Pain should be elicited in at least 11/18 established tender joints when a digital pressure of 4 kg is applied.

Physiotherapy management :-
- Initial phase :- treat the underlying cause like sleep disorder, infection, connective tissue disorders etc.
- Second phase :- Myofascial release, massage & manipulation .
- Final phase :- Aerobic exercises are given to improve the muscle endurance.

🌀Edema is swelling in the body caused by excess fluid. It often affects the lower body, such as the legs, feet, and ankles, but it can occur anywhere. If you press on a swollen area and an indentation or pit remains, it’s called pitting edema. If there’s enough fluid, and the swelling is from fluid and not something else, then most edema will pit.

Pitting edema is either caused by a localized problem with veins in the affected area, or a systemic problem with your heart, kidneys, or liver function.

Symptoms:-
-abnormal cough
-chest pain
-shortness of breath
-heart murmur
-low blood protein, especially if the swelling is in your abdomen
-decreased stamina
Symptoms occur case by case, depending on what’s specifically causing the edema.

Causes:-
Pitting edema is a general problem caused by a variety of issues such as:
heart valve problem
slow protein levels
deep venous thrombosis(DVT)
—blood clots,
usually in the legs
severe lung disease
congestive heart failure
venous insufficiency
liver disease
kidney failure
obesity
pregnancy

Diagnosis:-
- Patient present with a history of severe trauma followed by pain, swelling & deformity.
- Associated with a shortening of the leg.
- One may be able to feel the head of the femur in the gluteal region.
💊💊💊💉💉💉💉💉Treatment :-
Reduction of a dislocated hip is an emergency, since longer the head remains out, more the chances of it becoming avascular.
Technique of closed reduction :-
- The patient is anaesthetised & placed supine on the floor.
- An assistant graps the pelvis firmly. The surgeon flexes the hip & knee at a right angle & exerts an axial pull.
- Usually one hears a sound of reduction, after which it becomes possible to move the hip freely in all directions.
- The leg is kept in light traction with the hip abducted, for 3 weeks. After this hip mobilisation exercises are initiated.

Complications :-
a) Injury to the sciatic nerve : behind the posterior wall of the acetabulum.
b) Avascular necrosis of the femoral head :- On x ray 1 - 2 years after injury dense, collapses, pain.
c) Osteoarthritis :- in congruous acetabulum & femoral head, Total hip replacement
d) Myositis ossification :- Pain, stiffness, fluffy new bone around the hip.
