You are the doctor in charge of the medical tent at the City Marathon. There has been a record turnout, and uncharacteristically for Ireland, it’s a sunny, roasting day. For the first couple of hours of the race, you and the first-aiders treat some blisters, sprains and even a case or two of sun-burn.

The first patient brought to the majors area arrives two hours into the race, by which stage the sun is blazing. They are weak and mumbling incoherently with red warm skin. The medical team measures their temperature at 40.5 C
