It was a great event in collaboration with RCSI Emergency Society! Participants got hands on experience to learn about the clinical applications of ultrasound in medicine.

We are so grateful to everyone who attended and we hope to see you all next time!

Welcome back to the DIMSS Digest, where we break down a high-impact clinical trial or guideline into digestible and clinically relevant teaching points โ€” helping you build the skills to interpret, critique, and apply real research to real patients.

This week: Inhaled Treprostinil for Idiopathic Pulmonary Fibrosis (TETON-2 Trial)

A new New England Journal of Medicine study evaluates whether inhaled treprostinil, a prostacyclin analogue with proposed antifibrotic effects, can slow disease progression in patients with idiopathic pulmonary fibrosis (IPF). In this phase 3 randomized controlled trial, treprostinil significantly reduced decline in forced vital capacity and lowered rates of clinical worsening compared to placebo over 52 weeks, highlighting a potential shift toward targeted, inhaled therapies in fibrotic lung disease. Check out our Google Drive for a full breakdown of the trial, created by the DIMSS Education Team.

Stay tuned every two weeks as we continue unpacking the studies shaping internal medicine โ€” so you can stay sharp for your next ward round.

This week: AI-Driven Clinical Decision Support in Acute Ischaemic Stroke

A new BMJ original article explores the clinical benefit of an AI-integrated Clinical Decision Support System (CDSS) for acute ischaemic stroke care compared to usual care, which is the current clinical standard, and investigates whether this digital intervention reduces the risk of new vascular events post-stroke. Check out our new Google Drive for the detailed breakdown of this trial, written by TCD Education Team Member, Kat.

This week: Trastuzumab Deruxtecan in Residual HER2-Positive Early Breast Cancer

A new NEJM original article explores the clinical benefit of Trastuzumab Deruxtecan as a postneoadjuvant therapy for HER2-Positive Breast Cancer compared to Trastuzumab Emtansine, which is the current standard treatment, and investigate which therapy increases the likelihood of invasive disease-free survival post treatment. Check out our new Google Drive for the detailed breakdown of this trial, written by Education Officer Raghav.

This week: Ianalumab plus eltrombopag in immune thrombocytopenia (VAYHIT2 trial)

This past 3 randomised trial evaluated whether a short course of Ianalumab, a B-cell-targeting monocloncal antibody, could improve durable disease control in adults with immune thrombocytopenia who relapsed or failed first-line glucocorticoids. The study showed that Ianalumab plus eltrombopag significantly prolonged time to treatment failure and increased sustained platelet responses compared with eltrombopag alone, suggesting a potential disease-modifiying approach that may reduce long-term dependence.
