Alexander Kielland capsize — 27 March 1980

On 27 March 1980, the semi-submersible accommodation platform Alexander Kielland capsized in the North Sea during a storm. 123 people died. It was the worst offshore disaster in Norwegian history.

The Norwegian investigation found that the capsize was initiated by the failure of a bracing member — a D-6 brace connecting one of the five supporting columns to the main structure. The brace had developed a fatigue crack over time, not detected during inspections. When it failed, the load transferred to other bracing members that were not designed to carry it alone. The collapse was rapid.

The loss of life was compounded by secondary failures in evacuation. The capsize was faster than most survival craft procedures were designed for. Lifeboat and life raft availability and deployment in the actual conditions were significantly worse than certification implied.

Three direct legacies for offshore safety:

First, offshore structural integrity inspection became a regulated discipline rather than a general engineering responsibility. The standards for inspection frequency, crack detection methods and structural fatigue assessment in offshore structures were substantially tightened in Norway and internationally following the disaster.

Second, evacuation system design standards were revisited. The time-to-capsize window that the Alexander Kielland demonstrated was used as a design criterion for survival craft that hadn't previously had to account for it.

Third, the concept of a Safety Case for offshore installations, requiring demonstration that risks have been reduced to as low as reasonably practicable, was accelerated in Norway by this disaster, alongside but independently of the UK's Piper Alpha response in 1988.

BFF (Basic Firefighting, incl. SCBA) Training – Delivered and Certified by Samson Tiara

Offshore and oil and gas safety training gets the most attention in discussions about workplace fire response. But the facilities where most workers in Indonesia spend their day, factories, shopping centres, hotels and schools, need trained fire response too.

Basic Firefighting training is for those environments.

BFF covers fire class identification (not all fires are fought the same way), correct extinguisher selection for the type of fire, practical extinguisher operation and the decision of when to fight a fire and when to get out. SCBA is included where the environment requires it.

The fire warden and fire response roles in commercial and industrial buildings are real positions with real responsibilities under Indonesian fire safety regulations. The people in those roles should know more than where the fire exits are.

A fire in a hotel kitchen is a Class F grease fire. Using a water extinguisher on it makes things significantly worse. A factory with flammable solvent storage needs personnel who can identify a Class B scenario and respond appropriately.

Basic Firefighting training is the starting point for any workplace that has people, materials and an obligation to respond if something ignites.

Yesterday was Indonesia's Independence Day. Today everyone goes back to work.

In the oil and gas sector, in petrochemical plants, on offshore platforms and in the industrial facilities that keep the economy moving, the work went on through the holiday and the work continues today. That's the nature of continuous process industries.

There's something worth acknowledging in that.

Heat stress monitoring — WBGT and work/rest ratios

Working outdoors in Indonesia in August means heat stress is a real and manageable occupational risk. Here's how to manage it properly:

1️⃣ Use Wet Bulb Globe Temperature (WBGT), not just air temperature. Air temperature alone is a poor predictor of heat stress risk. WBGT accounts for humidity, radiant heat and air movement, which are the variables that actually determine how much thermal stress the body is under. The threshold for moderate work under WBGT guidance is around 28°C. Heavy work drops that threshold further.

2️⃣ Acclimatisation takes time. Unacclimatised workers are at significantly higher risk in the first 7 to 14 days of working in a hot environment. New crew members, workers returning after leave and anyone moving from an air-conditioned environment to outdoor work need a structured acclimatisation period, not immediate full workload.

3️⃣ Work/rest ratios are not optional. In high WBGT conditions, structured rest in shade or cool areas is a control measure, not a convenience. The specific ratios depend on WBGT, workload and acclimatisation status. NIOSH provides guidance tables.

4️⃣ Early warning signs matter more than late ones. Fatigue, irritability, reduced concentration and headache are early heat stress indicators. By the time someone has heat exhaustion or heat stroke symptoms, the situation has already escalated to an emergency.

5️⃣ Hydration is a baseline, not the whole answer. Adequate fluid intake is necessary but not sufficient. The other controls — WBGT monitoring, acclimatisation and work/rest ratios — are not replaceable by drinking more water.

CSEA (Confined Space Entrant & Attendant) Training – Delivered and Certified by Samson Tiara

In confined space operations, most of the focus goes on the entrant. What they're doing inside, whether they're okay, whether they need to come out.

The attendant is the more important role.

The attendant doesn't enter. They monitor. They communicate. They maintain the entry log. They track time and atmospheric conditions. And critically, they are the first line of response when something goes wrong inside.

What many confined space incidents have in common is an attendant who left the position, lost communication with the entrant or attempted entry themselves when the entrant stopped responding. That last action, a well-meaning attendant going in to help, is the mechanism behind the statistic that over 60% of confined space fatalities are would-be rescuers.

The correct response when an entrant is in distress is non-entry retrieval and emergency services activation. Not entry.

Discussion: Has pressure from a supervisor or client ever caused you to compromise on a safety standard or procedure? What happened?

This is a question most safety surveys collect anonymously, if at all, because the honest answer is professionally sensitive.

The pressures can take different forms. Sometimes it's explicit: ""We need to get this done before the weather window closes."" Sometimes it's implicit: a supervisor who doesn't say anything but whose history makes clear that delays won't be received well. Sometimes it's structural: a contract clause that penalises downtime, a client relationship that depends on not raising problems.

The accommodation can also take different forms. A pre-task assessment that's completed faster than it should be. A stop work that isn't called because calling it last time led to a difficult conversation. An inspection that's signed off based on the previous shift's word rather than a physical check.

Nobody in any of those situations necessarily made a decision they understood to be obviously wrong. The pressure tends to work on the margins of what counts as acceptable rather than on the bright line between safe and unsafe.

AGT-OPITO (Authorized Gas Tester) – OPITO Approved by Samson Tiara

Atmospheric gas testing is one of the most critical elements of the Permit to Work system, and one of the most frequently misunderstood.

The AGT role isn't to take a reading and note it on the permit. It's to develop a sampling strategy that reflects the task, the work environment and the probable gas distribution, take readings that are actually representative of the hazard and interpret those readings correctly against the conditions at the time of the work.

That's a more demanding role than a single reading implies.

In our OPITO Authorized Gas Tester course, we cover sampling strategy as a core element. Where to sample in a complex space, how to sample for gases that are heavier or lighter than air, how to account for pockets and dead zones and how to re-test when conditions change during the work.

We also cover instrument use and limitations: the difference between instruments, what each detector actually measures versus what it's assumed to measure and how temperature and humidity affect readings.

Gas testing isn't background activity. It's a technical decision-making process that directly determines whether work proceeds safely or not. The AGT who understands their role changes how that process works on a worksite.
