The primary role of the sports medicine physician in competitive sport is the comprehensive health management of the elite athlete to facilitate optimal performance. This can be achieved with an appropriate focus on the physician’s circles of clinical competencies and will include pre-participation health evaluations, integrated electronic medical records, health and performance-focused risk decision making, a focus on optimising performance and a thorough understanding of the integration of communication and implementation strategies within the particular sport.
Our evolving specialty is facing huge challenges but also exciting opportunities. We will be better positioned as sports medicine physicians to deal with these when we accept the fact that we ‘first need to cure ourselves of the doctor’s disease – the pathological compulsion to cure’.
The role of the sports medicine physician can be an extraordinarily rewarding one. We can work with highly talented, interesting, varied and inspirational athletes and coaches (from the academic to the artisan!); a rich tapestry of personalities that many other medical professionals could never collaborate with as colleagues in the traditional medical world. We should run close to the athlete and coach, managing health, optimising performance and leading the way in collaborative, balanced, evidence- and preference-based, individualised healthcare.

題目: 跑步競賽訓練注意事項

近年跑步運動漸漸成風,不少跑友更加會參加各式賽事,強身健體之外亦可以磨練意志。
「磨練意志」固然是重要的一環;很多跑友仍然認為要追求身心突破 (跑友間稱之為 Mind over Body),忍耐痛苦是必然的,甚至企圖強行超越身體承受能力,最後練得一身傷患,得不嘗失。
我們確實見過不少跑步傷患,病灶並不只在於身體勞損。最大的病灶,還是植根已久的錯誤訓練觀念。今天撰文,希望大家認真看待跑步運動,不要「下下靠估」。

悠閒跑步,當然可以按自己安排來運動。然而當跑友定下目標參加了比賽,這種隨意訓練方式,不但跑得沒有效率,更有機會「跑壞腳」。畢竟報了比賽,理應認真參與和計劃,在一眾競賽跑友之間將自己發揮得最好,才能體驗體育精神。
系統化訓練
不同跑步教練,對制定訓練系統都有不同意見。然而各種系統之中,都離不開基本科學原則 – F**T 原則,訓練項目設定原則,心率訓練方案,以及賽前調整。
F**T 原則,即是訓練密度 (Frequency),訓練強度 (Intensity),訓練項目 (Type),以及訓練時間 (Time)。要有系統記錄訓練進度,必然離不開 F**T 原則,而增加或減少訓練時,亦應該每項分別審視,找出其中最關鍵一環 (key factor),訓練才會有效率。
訓練項目 (Type) 中,亦有細分專門性 (Specificity),負重性 (Overloading),進階性 (Progression) 和訓練週期調整 (Periodlization) 等,應該交由專業跑步教練設定。
而心率訓練方案和賽前準備 (例如心跳錶訓練設定,醣原負荷法等),坊間已有不少資訊,當中有對有錯,有疑問時應向教練查詢。
跑姿調整
物理治療師對步履和跑步姿勢,有一套科學化檢測系統。
要詳細理解跑姿,要從跑步步態週期 (Running gait cycle) 說起。簡單來說,要了解跑友的跑姿,我們要得到完備的資料:
- 左右下肢平衡
- 支撐期 (stance phase) / 擺動期 (swing phase) 分佈
- 髖 / 膝 / 踝 / 足關節的力學
- 接地點 (contact point) / 接地時間 (contact time)
- 重心 (centre of mass) 的活動
人類的視力有限,單靠肉眼,絕不可能詳盡分析一個跑姿。物理治療師會從各項影片,相片,觀察,器材檢測中,分析出跑姿概況 (running form profile)。當然,有經驗的教練,亦可以直接從觀察中找出問題所在。
訓練回復方案
跑步訓練,並非單純硬性訓練累積 (Work hardening),而是有系統的生理調整,將身體各樣機能發揮得最好,達致最理想的均衡狀態。有效和恆常運動,已經可以增加基礎代謝;加上足夠的休息和回復,身體「出入平安」,不會做成入不敷支的透支狀態,訓練便會健康精進。
現時流行速效回復 (Active recovery),包括冰療、壓力治療、泡沫軸治療,或甚不同食用補充劑,鮮有臨床實證;我的個人觀點,只要方法是安全及經廣泛應用,經濟和時間上都可以應付的話,跑友可以自行嘗試,客觀判斷。

題目: 甚麼是隨行隊醫 / 軍醫

電視直播體育賽事,不時會見到軍醫跑出賽場,噴『神仙水』和出動擔架。這可能是大部分觀眾對隨行隊醫的唯一印象。隨行隊醫工作多元化,傳媒中見到的臨場支援 (on-field support),其實只是整個隨行隊醫概念之中的一角,卻又是最吸引眼球的一。
隨行隊醫的人員資格,並沒有統一標準。在香港,普遍隨行隊醫都是醫生及物理治療師;在美國,普遍隨行隊醫是運動傷害防護員 (Athletic Trainer)。
隨行隊醫,並不應該只出現在競賽場地。隨行隊醫,是真正的隨行,在休季時為運動員做好預防方案,在賽季時為運動員提昇回復及治療各種傷患 (sports-specific) 的功能訓練方案,為運動員及運動團隊製定長期觀察,長期計劃和長期治療 (longitudinal observation / planning / treatment)。
這種長期照料,對運動員的競賽生涯是必須要的。可知道精英運動競技的可容許錯誤 (margin of error) 是相當相當少,隨行隊醫花一個月去預備,並讓運動員適應下來,絕對是正常不過。更甚的是,隨行隊醫必定會於出發前準備好每位運動員的物資清單,一來隊醫可以提早準備,二來可以安排一份備用物資予運動員。就算隨行隊醫有任何突發事故而未能到場都好,運動員都不會處於毫無準備的情況下比賽。隨行隊醫更加會擬定不同後備方案,運動員舊患復發也好,新傷也好,都可以即時檢查處理。假若到臨比賽前才檢查這檢查那,時間不足,對運動員的了解不足,醫護人員根本不可能提供任何有效協助。
說到這裡,我想重申一點,就是隨行隊醫的崗位,必須由一位肯付出時間心力作長期觀察、長期計劃和長期治療的醫護人員來擔當。
『主治治療師制度』
這個制度,是職業競賽中必然存在的一環。當運動員受傷時,必須到團隊指定的『主治治療師』處檢查確診。『主治治療師』會就臨床檢查結果,將運動員安排入健康名單 Fit List / 觀察名單 Watch List / 傷患名單 Injury List 中,再為其訓練,比賽及治療方案作不同安排。為表公正,傷患表格 Injury List 會以一聯三份形式,治療師,教練及運動員各持一份。主治治療師會為運動員提供合適的治療及轉介。
大家看來可能覺得太複雜,甚至有點無謂。然而當考慮到傷患情況跟團隊目標,甚至運動員自身的前途的密切關係,公開公正就是對三方最大的保障。這份傷患史,也會跟著運動員,甚至轉到不同球會,成為運動員的 portfolio。
當運動員希望選擇主治治療師以外的治療師或治療方式,他/她必須先徵詢主治治療師的意見;過了主治治療師一關後,又要得到教練和領隊的同意,才可以正式出外就診。這是確保公正之餘,更加可以保護運動員的運動生涯;畢竟藥石亂投的後果可以相當嚴重。
在英國有一個業界慣例, 當治療師在執業診所發現其他治療師主治團隊的運動員掛診,他/她會直接向運動員查明授權狀況,亦會致電向主治治療師查詢。 這是專業禮儀之餘,更加可以確保專業資訊互通,為運動員提供最好方案。
在香港,要執行『主治治療師制度』還是非常困難,我們拒絕了未經授權的外隊運動員掛診之餘,也見到不少運動員出外尋醫,而外診治療師也沒有實行通報機制,結果『長期觀察,長期計劃和長期治療』就不能貫切實行。畢竟要實行一個制度, 總要有先行者,診所生意可能會少了,不過這是我們尊重『主治治療師制度』的承擔。 希望各位管理人員, 運動員和物理治療師明白我們的心意。

Youth sports injury prevention: keep calm and play on
In recent years, there has been extensive media focus on youth sports injuries, especially concussions. While youth sports training and competition are more intense than ever, the focus on the ‘injury risk’ downside of youth sport is unfortunate because the benefits of sports participation (improvements in overall health and bone density; lower rates of overweight/obesity, cardiovascular disease, diabetes, risk-taking behaviours, depression and teen pregnancy; and enhanced self-esteem and peer socialisation) far outweigh the risks for children and teens.
In fact, the injury risk for youth aged 6–12 years in organised sports, even in contact/collision sports such as American football, is well below the risk of injury in typical recreational activities such as riding a bicycle or playing on a playground.
