The Rosie Young Lectureship was established in 2024 in honour of the remarkable contributions made by Professor Rosie Young. This lectureship aims to uphold the high standards of teaching, clinical practice, and research in the field of Internal Medicine.

Professor Mak’s research interests include clinical liver diseases, especially in the field of chronic viral hepatitis. She has published extensively in leading journals of the field, and her work has been cited in international clinical guidelines and led to advancement in development of novel drugs in clinical trials. Professor Mak has also spearheaded key public health initiatives, including leading a Hepatitis C virus micro-elimination effort, pioneering such strategy in persons under custody in correctional facilities.

Congratulations once more to Professor Mak on this achievement!

A research team led by Professor Anskar Leung Yu-hung, Chair Professor in the Department of Medicine, School of Clinical Medicine, HKUMed, has developed a novel combination therapy that significantly improves treatment outcomes and survival in patients with acute myeloid leukaemia (AML) with FLT3 gene mutations. The study found that the combined use of the FLT3 inhibitor Quizartinib and the protein synthesis inhibitor Omacetaxine Mepesuccinate (collectively termed QUIZOM) effectively suppresses the growth of cancer cells and activates the patient's immune system, achieving a composite complete remission (CRc) rate of about 83% while reducing the risk of relapse.

The QUIZOM combination therapy has completed a Phase 2 clinical trial, with findings corroborated through multi-omics mechanistic studies. This breakthrough represents a significant advance in the treatment of high-risk blood cancers.

【最新消息】港大醫學院研究團隊開發新型混合療法 大幅減低白血病復發率 爭取時間作骨髓移植

港大醫學院臨床醫學學院內科學系研究團隊開創新型混合療法，能夠顯著改善帶有FLT3基因突變的急性骨髓性白血病（AML）的治療成效，並提高患者存活率。研究發現，聯合使用FLT3抑制劑Quizartinib與用於慢性白血病的蛋白質抑制劑Omacetaxine Mepesuccinate（合稱QUIZOM），不僅有效抑制癌細胞生長，更能激活患者自身免疫系統，令完全緩解率提高至約83%，同時降低復發風險，QUIZOM混合療法目前已進行第二期臨床試驗，並獲得多組學機制研究的驗證，為治療高風險血癌帶來突破性進展。

混合療法提升緩解率至八成以上

急性骨髓性白血病是一種致命率極高的血液腫瘤，其中FLT3是最常見的基因突變，約佔所有急性骨髓性白血病個案的三成，復發率極高，預後普遍不理想。儘管現行的FLT3抑制劑能短暫改善療效，但未能解決高復發率的問題，患者往往需要盡快進行異體造血幹細胞移植，否則病情容易惡化。

由港大醫學院臨床醫學學院內科學系講座教授梁如鴻教授領導的研究團隊，於2017年11月至2020年9月期間，進行一項二期臨床研究，共招募40位年齡介乎23至81歲、帶有FLT3基因突變而且對化療耐藥的白血病患者接受QUIZOM混合療法。結果顯示，患者的綜合完全緩解率達至約83%，中位無白血病存活期為10個月，中位總存活期為12. 9個月；當中13名患者在治療後，更成功接受異體造血幹細胞移植。

梁如鴻教授表示：「QUIZOM混合療法為不適合常規化療的FLT3急性骨髓性白血病患者提供一個高效而可行的治療選項，透過提高緩解率，讓患者得以銜接骨髓移植以鞏固治療，再配合移植後的治療及監察，大部分病人的病情可以持續緩解。」

揭示耐藥關鍵 克服復發難關

為深入理解治療機制，研究團隊進一步進行多組學分析，首次揭示QUIZOM的核心效用機制。研究團隊透過單細胞基因表達分析，發現QUIZOM混合療法不單破壞癌細胞的蛋白質代謝，抑制白血病細胞生長，更激活了患者身上的T細胞免疫系統。研究顯示，激活免疫系統對治療效果發揮重要作用，產生類似化療結合免疫治療的雙重效益。

此外，團隊在小部分復發患者的白血病細胞中，發現了一群具有幹細胞特性的耐藥細胞。這些細胞透過PLD1酶驅動的磷脂代謝，促進蛋白質摺疊，從而產生耐藥性，導致疾病復發。研究證實，加入PLD1抑制劑可有效抑制這類白血病耐藥幹細胞的再生功能，並增強治療效果。

專利申請為未來臨床應用鋪路

梁如鴻教授表示：「這項研究不僅在臨床上證實QUIZOM的應用潛力，令更多高風險患者適合接受造血幹細胞移植，更全面闡明其治療效應，為未來克服白血病耐藥性提供清晰而具潛力的治療方向。研究團隊已就PLD1抑制劑在白血病上的發現申請專利，期望未來能進一步改善急性白血病的治療方案，惠及更多血液病患者。」

The 30th Hong Kong Medical Forum (HKMF) held over last weekend was a resounding success, attracting over 2,700 participants. This year, the Forum was held at a new location, the HKCEC Convention Hall, enabling the accommodation of a larger audience and the introduction of parallel sessions for the first time in HKMF history. This innovative format allowed a diverse range of speakers from different backgrounds to share insights, spark discussion, and foster new partnerships with attendees. In celebration of the forum's 30th anniversary, keynote presentations from over 40 overseas and local speakers - the largest number in HKMF history - were held, as well as an exhibition of the latest and greatest medical product and innovations.
