Aspiration is one of the techniques practitioners may use as part of a wider strategy to reduce vascular risk when injecting dermal filler.

When treating the nasolabial folds, a detailed understanding of facial anatomy is essential. The facial artery and its branches can have significant anatomical variation, which means safe injecting cannot rely on a single technique alone.

During training, we teach practitioners the principles behind aspiration, including how it is performed, its limitations, and how factors such as needle position, syringe and product characteristics can affect its reliability.

Importantly, a negative aspiration does not guarantee that the needle tip is outside a blood vessel. Aspiration should therefore never create a false sense of security.

Safe filler practice requires a combination of:
• Detailed anatomical knowledge
• Appropriate product placement and depth
• Controlled injection technique
• Small, considered aliquots where appropriate
• Continuous observation of the tissue and patient

One of the most common mistakes we see in aesthetics training is injecting directly through your skin markings.

When you mark up a patient's face, those markings are sitting on the surface of the skin and can harbour bacteria, makeup residue, oils, and contaminants. By passing your needle through the marker ink, you risk introducing these contaminants into the tissue, increasing the risk of infection and compromising your infection control standards.

Instead, always: • Mark your treatment areas first • Clean away the injection point if required • Inject adjacent to the marking rather than directly through it • Maintain strict aseptic technique throughout the procedure

Good aesthetics is not just about beautiful results. It's about patient safety, clinical standards, and attention to the small details that reduce risk.
