Dear Parents and Carers,

As part of our commitment to reducing paper waste and creating a more efficient way of documenting information about your children, we are introducing a new digital Pre-Existing Injury Form.

If your child arrives at nursery with an injury that occurred at home, you can simply scan the QR code displayed at the entrance (or before you arrive) and complete the form on your phone. Once submitted, the form is sent directly to the nursery managers' email, allowing us to record the information securely on our system.

There is no longer a need to complete a paper form—just scan, complete, and submit.

If you do not have your phone with you, a nursery iPad will be available for you to use.

Please note that the form must be completed before you leave the nursery. As part of our safeguarding responsibilities, we are legally required to keep an accurate record of any injuries that children arrive with. Recording pre-existing injuries at the time of arrival helps us distinguish between injuries sustained before your child came into our care and any incidents that may occur while they are at nursery. This ensures clear, accurate records and helps protect the wellbeing of every child, as well as supporting our legal and regulatory responsibilities.

Thank you for your support as we introduce this new process. We hope this change will make reporting pre-existing injuries quicker and easier for everyone.
