PLAB 2: Medication Overuse Headache (Simple Summary)

1. Understand the Original Headache

* Ask how the headaches started.
* Check if they’ve changed in type or frequency.
* Identify previous headache disorders (e.g. migraine, tension headache).

2. Check for Medication Overuse

* Ask what painkillers are being used.
* Ask how often they’re taken.
* Using painkillers more than 2–3 days a week may suggest medication overuse headache.

3. Look for the Underlying Cause

* Medication overuse is often secondary to another headache problem.
* Consider migraine, tension headache, stress, poor sleep, anxiety, caffeine, or lifestyle factors.
* Screen for red flags.

4. Management

Do:

* Explain that overusing painkillers can worsen headaches.
* Stop the overused simple painkiller.
* Warn headaches may temporarily worsen before improving.
* Treat the root cause.
* Address stress and lifestyle.

Do Not:

* Assume this is the original headache disorder.
* Keep prescribing more painkillers without addressing overuse.
* Ignore red flags or lifestyle factors.

5. Safety Net & Follow-Up

* Seek urgent help for sudden severe headache or neurological symptoms.
* Arrange follow-up to monitor withdrawal and reassess diagnosis.
* Consider prevention strategies and lifestyle advice.
