Anatomical ageing ≠ Treatment target.
One of the assumptions in aesthetic medicine is that if ageing causes an anatomical change, we must somehow replace or reverse that change.
But do we?
Bone remodels.
Deep fat compartments change.
Ligaments alter.
Subcutaneous tissue redistributes.
The dermis deteriorates.
Yet successful rejuvenation does not necessarily require us to reconstruct a 20-year-old anatomy.
Our objective is different: manage the visible expression of ageing.
That may mean improving the dermis, strategically restoring superficial contour, and replacing projection or support at the periosteal level—without necessarily filling every deep fat compartment simply because anatomical ageing has occurred there.
After two decades of injecting, I increasingly think of aesthetic treatment as strategic camouflage rather than anatomical reconstruction.
Treat what contributes meaningfully to the appearance—not every structure that happens to have aged.
Ageing is anatomical.
Aesthetic treatment is perceptual.
Regenerate. Support. Camouflage.
