Aims of Respiratory Physiotherapy

Let’s start right at the beginning. Physio’s don’t just do bones and muscles - what?!

We also treat impairments, disease and injuries to patient’s cardiovascular and respiratory systems. This is particularly important somewhere like my current rotation on critical care where lots of my patients are intubated and ventilated. We have a crucial role in maintaining the health of these patients who often can’t cough and clear their chests themselves, especially if they are sedated and paralysed to tolerate the ventilator. If they’re unable to clear their own secretions, these can build up and lead to nasty chest infections. This is where physio’a come in.

We usually think of the key 4 impairments which are amenable (effected by) physiotherapeutic interventions:

🫁 Reduced lung volumes
🫁 sputum retention
🫁 increased work of breathing
🫁 Reduced exercise tolerance

CHEST TRAUMA PART-2

I’ve have highlighted the 4 most common types of chest trauma that I’ve seen so far in ICU and their specific management steps.

One thing to note: when using positive pressure in someone with rib fractures, make sure the ribs aren’t majorly displaced, and double check there’s no pneumothorax first. We don’t want to puncture any lungs, or make a pneumothorax worse ❌

Chest Trauma (part 1)

The ribs and chest are vital in protecting the organs underneath, but also need to be able to move to let us breathe effectively. This means they have weak points and so are often injured which can lead to further medical complications - to be outlined in the next post.

Therefore, a lot of my patients have some sort of rib fracture, pneumothorax or lung contusion as a result of their accident or injury.

Broken ribs are incredibly painful, as well as impacting thoracic expansion/ our breathing pattern. This means we are less keen to take a big deeper breath, or cough which can lead to respiratory complications such as retained secretions, atelectasis or rescued volumes. This is where chest physio comes in, to help our patients to manage their symptoms to prevent complications, or to help them to get better if those complications occur anyway.

Mobilising is the gold standard, otherwise positive pressure (such as IPPB or the cough assist) is another tool in the physio toolbox I often use. - but may be a precaution or contradiction in some chest trauma patients! Any management will be patient specific for this reason 🫁

One of the most important things to know for any MSK assessment is key signs which may indicate a serious pathology, such as cauda equina.
