🔎 Could home-based decongestion redefine the management of worsening heart failure?

Presented at HFA 2026, the SUBCUT-HF II trial explores whether early discharge with an ambulatory, patient-managed subcutaneous furosemide pump can optimize heart failure care.

In this new Trial Review, Jan Biegus reviews the evidence and explores the potential of this innovative home-based decongestion strategy for patients with worsening heart failure.

👀 Do you remember the case of a 39-year-old patient presented with a late STEMI complicated by severe biventricular dysfunction and a non-restrictive ventricular septal defect (VSD) rapidly requiring peripheral VA-ECMO and IABP support?

Faced with this life-threatening scenario, opinion was highly divided among respondents. Discover now the Heart Team's strategy to manage this major mechanical complication.

🫀 or ? The answer isn't always straightforward.

For patients with and chronic coronary syndromes, choosing the optimal revascularisation strategy goes far beyond coronary anatomy.

In this excerpt from our last iHF Webinar, experts discuss the respective roles of CABG and PCI, the importance of achieving complete revascularisation, and why every complex case should be reviewed by a multidisciplinary Heart Team.

🫀 Can therapy redefine treatment for a specific phenotype?

Published in Circulation, the PRAISE-MR trial explores whether sacubitril/valsartan can improve haemodynamics, exercise capacity and atrial functional mitral regurgitation in patients with HFpEF.

In this new Journal Club, Jolie Bruno reviews the study, highlights its key findings, and discusses what they could mean for the management of HFpEF in clinical practice.

🫀 Post-MI ventricular septual rupture: a critical trans-disciplinary case

A 39-year-old patient with no medical history presented with a late STEMI, severe biventricular dysfunction, and a non-restrictive ventricular septal defect (VSD).
Despite urgent PCI and IABP support, his condition quickly deteriorated, with severe lactic acidosis and pulmonary oedema. Intubated and transferred to a hub centre, he required peripheral VA-ECMO rescue.

How would you manage this post-MI mechanical complication?
