Applications are accepted Sunday to Thursday, from 9:30 am to 1:00 pm.
To apply, please bring the following documents:
A copy of your child's birth certificate
Copies of both parents' IDs
A passport-sized photo of your child
Note: Applications MUST be filled by a parent or guardian.
