🦷 The loss of five anterior teeth combined with severe alveolar bone deficiency presents a clinical situation in which the outcome depends on close coordination between every stage of treatment.
This case reviews the multidisciplinary rehabilitation of a patient following severe trauma, including diagnosis, treatment planning, orthodontic preparation, bone grafting, implant placement, and the 2-year outcome.
The authors place particular emphasis on implant positioning, occlusal conditions, support of the subnasal region, and long-term esthetic risks in the anterior maxilla.

What Determines the Stability of the Gingival Margin After Root Coverage?
When treating gingival recessions in the esthetic zone, it is important to evaluate not only the depth of the defect and the ability to advance the flap coronally.
For long-term stability, the following factors are essential:
🔹 soft tissue thickness;
🔹 width of keratinized tissue;
🔹 flap stability;
🔹 quality of graft fixation;
🔹 tension-free flap closure.
A connective tissue graft should not be used by default. It is indicated when there is a need to improve the biological conditions for healing: to increase tissue thickness, support the gingival margin, and reduce the risk of recurrent apical displacement.
In this post, we discuss when CTG is particularly justified and which factors should be considered before selecting a surgical approach.

🦷 A fractured instrument in the root canal is not always an indication for immediate removal.
What matters far more is determining which approach will truly improve the tooth’s prognosis: removing the fragment, bypassing it, or avoiding aggressive intervention altogether.
Today, endodontics relies increasingly not on technical skills alone, but on a precise analysis of the clinical situation. CBCT findings, root canal anatomy, the amount of dentin that would need to be removed, the location of the fragment, and the degree of infection beyond it are the factors that ultimately determine both the treatment strategy and the long-term outcome.
🔬 In his research, Yoshi Terauchi proposes viewing fragment retrieval not as an isolated procedure, but as a decision-making algorithm in which the following factors play a key role:
✔️ visibility under the microscope
✔️ the degree of canal curvature
✔️ the length of the fractured instrument
✔️ the biological prognosis of the tooth
In this carousel, we discuss when an attempt to remove the fragment is truly justified, when bypass is the preferred option, and why, in some cases, leaving the fragment in place may be the most rational decision.
