Ultrasonographic examination reveals a large adrenal gland mass with suspected invasion into the adjacent caudal vena cava. Intraluminal extension of tumor tissue is visible within the vena cava, consistent with a tumor thrombus. These findings are highly suggestive of an invasive adrenal neoplasm with vascular involvement.

🐝 When curiosity stings...

This young dog was presented to our clinic after suddenly screaming whenever someone touched her head, drooling excessively, and refusing to eat. The owner told us the symptoms started shortly after playing in the garden.

During the examination, the only abnormal finding was mild tachycardia. A careful oral examination revealed the cause: a honeybee stinger still embedded inside the mouth, with attached venom sac and tissue.

Young puppies are naturally curious. They often try to investigate—or even eat—bees. Unlike wasps, honeybees leave their stinger behind, continuing to release venom for a short time after the sting.

A bee sting inside the mouth is more than just painful. It can cause:
Severe pain and excessive drooling
Swelling of the tongue and throat
Enlarged regional lymph nodes
Difficulty eating or breathing
Allergic reactions, including life-threatening anaphylaxis

If your dog suddenly develops facial pain, excessive drooling, or difficulty eating after being outdoors, don't ignore it. A thorough oral examination can reveal a hidden cause and allow prompt treatment.

Fortunately, this patient recovered well after the stinger was removed and appropriate treatment was started. ❤️

Thoracic Lymphoma in a Cat – Surgical Excision of the Sternal Lymph Node

A cat was presented with a history of persistent coughing. Thoracic radiographs revealed a soft tissue mass located cranial to the heart, consistent with enlargement of the sternal lymph node.

Advanced imaging with CT and fine-needle aspiration both raised a strong suspicion of lymphoma. To obtain a definitive diagnosis and reduce the mass burden, a surgical excision of the affected sternal lymph node was performed.

A parasternal approach provided excellent exposure. Careful tissue dissection was carried out using electrocautery for precise hemostasis and sharp dissection with iris scissors to preserve the surrounding structures. The enlarged lymph node was removed completely (en bloc).

Histopathology confirmed lymphoma.

This case highlights the importance of combining diagnostic imaging, cytology, surgery, and histopathology to reach a definitive diagnosis and guide further treatment. Complete excision also provides a much larger tissue sample than fine-needle aspiration, allowing accurate lymphoma classification and planning of chemotherapy when indicated.

Mitral Valve Prolapse vs. Mitral Valve Insufficiency – What's the difference?

These two terms are often used together, but they are not the same condition.

Mitral Valve Prolapse (MVP):
The mitral valve leaflets bulge back into the left atrium during heart contraction. This is a structural change and does not always cause blood leakage.

Mitral Valve Insufficiency (Mitral Regurgitation, MR):
The mitral valve does not close properly, allowing blood to flow backward into the left atrium. Over time, this can enlarge the heart and may eventually lead to congestive heart failure.

Important:
A dog can have a mitral valve prolapse without significant regurgitation, while many dogs with chronic degenerative mitral valve disease develop both prolapse and mitral regurgitation.

Early echocardiographic screening helps detect these changes before clinical signs appear, allowing appropriate monitoring and treatment when needed.

Pyogranulomatous Mastitis, Lymphadenitis and Thrombocytopenia

This dog was presented with a painful swelling of the right caudal mammary gland, markedly enlarged regional lymph nodes, fever, lethargy, and thrombocytopenia.
Given the extent of the lesions and the concern for systemic disease, surgical excision of the affected mammary tissue, enlarged lymph nodes, and severely enlarged tonsils was performed.
The presence of thrombocytopenia increased the risk of intraoperative bleeding. Therefore, advanced electrosurgical vessel-sealing and coagulation techniques were used throughout the procedure to achieve meticulous hemostasis and minimize blood loss.
Histopathology revealed severe pyogranulomatous and suppurative inflammation affecting the lymph nodes and tonsils. Special stains showed no evidence of fungal organisms, parasites, or acid-fast bacteria. While the exact underlying cause remains undetermined, the findings are consistent with a severe systemic inflammatory or infectious process.
This case highlights the importance of evaluating mammary gland swellings carefully, as they may represent only one manifestation of a much more widespread disease process.

Ear mites may be tiny, but they can cause significant discomfort.
Cats affected by ear mites often present with intense itching, head shaking, and a characteristic dark "coffee-ground" ear discharge. Diagnosis is typically straightforward with otoscopy and microscopic examination. Prompt treatment and management of all in-contact pets are essential to prevent reinfestation and secondary ear infections.

Ear Polyps in Cats

Feline ear polyps are benign inflammatory masses that commonly arise from the middle ear or Eustachian tube. Affected cats may present with chronic ear infections, head shaking, scratching, pain, balance disturbances, or even upper respiratory signs.

This video demonstrates removal of an ear polyp using the twist-and-pull technique. Although this method is often successful and minimally invasive, recurrence remains possible, particularly when the stalk extends deep into the middle ear.

Histopathological examination and follow-up examinations are recommended to confirm the diagnosis and monitor for recurrence.

Deep Paw Pad Laceration in a Dog
This dog was presented with a severe glass-related injury to the paw. The laceration was located on the medial aspect of the hind paw pad and extended deeply into the tissue, causing significant bleeding.
Paw pad injuries can appear small externally but often involve extensive underlying tissue damage. Due to the depth of the wound and ongoing hemorrhage, surgical treatment was required.
After thorough wound exploration, cleaning, and removal of damaged tissue, the defect was surgically repaired. Proper reconstruction of paw pad injuries is important because these structures are constantly exposed to weight-bearing forces and play a crucial role in locomotion.
Postoperative care includes bandage management, restricted activity, pain control, and close monitoring for infection or wound dehiscence.
Early veterinary intervention is essential for deep paw pad injuries to achieve the best possible functional outcome.
