โญ•๏ธAfter practicing implant dentistry for more than 15 years, you get a chance to see the good, the bad and the ugly in your own work. This time we are on lucks side with the good .
โญ•๏ธ3 year recall where we removed this upper aox due to a small chip in the zirconia to polish off allowing us to evaluate the tissue volumetric changes. I was very happy to see the volume maintenance achieved with PET that appears to only be better with time.
โญ•๏ธA combination of socket shield and root submergence techniques were used in this FP1 approach. As we evaluate our techniques , we can see what works and what doesnโ€™t. We have started to use more titanium substructures on our cases now.

โญ•๏ธOne of the most challenging implants is the single central incisor . Not only is surgical planning and tissue management critical , but also prosthetic and ceramic work to blend in after .
โญ•๏ธShowing a recent insert done at CIDN Digital Lab. A DESS USA Dental Smart Solutions ASC abutment was used to redirect screw channel.
โญ•๏ธSurgery performed by Dr Mark Bishara at Durham Dental Solutions. This failing upper central was replaced with an immediate Neodent Canada implant and same day temporary for 3 months prior to final fabrication.
โญ•๏ธThe patient was very happy with the esthetic result in this case.

โญ•๏ธShowing a recent insert at CIDN Digital Lab just in time for this lovely patientโ€™s daughters marriage . Part of our discussion on treatment planning at our AOX courses is how to choose when to go with removable snap in dentures versus fixed options .
โญ•๏ธFinancially , this patient could only afford snap in dentures but as a way to give back we upgraded him to fixed as a courtesy, as I felt he would have a much higher life satisfaction
โญ•๏ธIn general we try and direct the conversation for dentate patients towards fixed solutions as there are no guarantees of satisfaction with removable . For current denture patients , there are points of comparison: โ€œmy denture is loose, now stable or I wish to eliminate my dentures all together.โ€
โญ•๏ธAnother key element is number of implants to use. We generally aim for 5-7 implants in maxilla/mandible . If pterygoid fixation is achievable we do consider its use as well.
