Share your thoughts on gaps in community-based supports for people living with dementia - Survey for people living with dementia, mild cognitive impairment, or memory/cognitive challenges
Caregivers of people with dementia, mild cognitive impairment, or memory/cognitive challenges; Health care professionals Community service or social care providers; Researchers in the field of dementia.

We want to hear what community-based supports are most needed for people living with dementia and care partners.

If you’re a person living with dementia, care partner, researcher, or health/social-care provider, please complete our survey by June 19. This survey takes about 15-20 minutes to complete and is available in both English and French.

Your input will help identify gaps and inform future Dementia Community Investment proposals.

This study has been reviewed and received ethics clearance through a University of Waterloo Research Ethics Board REB # (48399).
