Retinal detachment is a detachment of the retina from the underlying pigmented choroid

There are different subtypes of retinal detachment

-Rhegmatogenous retinal detachment
-Retinal break
-Lattice degeneration
-Tractional retinal detachment
-Exudative retinal detachment

Rhegmatogenous retinal detachment is the most common in the 6th and 7th decade of life but can occur at any age and affects more male than female. It is most commonly caused by posterior vitreous detachment and trauma.

On ultrasound, retinal detachment appears as a bright, continuous, smooth and somewhat folded membrane within the vitreous, which is reflective and freely moving(less mobile than posterior vitreous detachment)on real-time imaging. Total or extensive detached retina has a typical triangular shape with insertion into the optic disc, such as the one in the clip attached.

Aside the obvious Retinal Detachment, what else do you see ?

Left Scrotal Discomfort.
Right Testicle Volume 14.96ml
Left Testicle Volume 7.06ml
Area in focus is left pampiniform plexus
Patient is supine and Acquisition is done on Valsalva maneuver.

Here I made a simulation of a typical pulse wave doppler artifact.

Image A showed an example of pulse wave doppler mirror image artifact. This is caused when the sample volume and angle correct cursor are not aligned with the direction of flow, but kept in position where it defects flow in both directions(antegrade, retrograde).

The mirror artifact is now corrected in B, by aligning the sample volume and angle correction cursor to a specific flow direction. The signal can be seen to show flow in the antegrade direction(forward flow)

Pulse wave doppler Aliasing is common problem usually encountered while interrogating vessels with pulse wave.

Image (a) showed a typical example of pulse wave aliasing.

PRF was too low = 1395.

This caused wrapping of the signal. As the sampled velocity is above the Nyquist limit .

In image (b) Aliasing has been corrected by raising the PRF value to 6250. This is a major effective solution to pulse wave aliasing.

The sampled signal must be consistently below the Nyquist limit.
