The leading cause of death among patients with life-threatening neurologic disorders remains withdrawal of life-sustaining measures due to a predicted poor prognosis. As evidence continues to evolve, clinicians are still faced with uncertainty, ethical complexities, and the challenge of balancing hope, suffering, and outcomes.

• The complexities and unknowns of the human brain
• Implicit cognitive biases in prognostication
