Lean systems approaches to health technology assessment: a patient-focused alternative to cost-effectiveness analysis.

Many countries now use health technology assessment (HTA) to review new and emerging technologies, especially with regard to reimbursement, pricing and/or clinical guidelines. One of the common, but not universal, features of these systems is the use of economic evaluation, normally cost-effectiveness analysis (CEA), to confirm that new technologies offer value for money. Many have criticised these systems as primarily being concerned with cost containment, rather than advancing the interests of patients or innovators. This paper calls into question the underlying principles of CEA by arguing that value in the healthcare system may in fact be unconstrained. It is suggested that 'lean management principles' can be used not only to trim waste from the health system, but as a method of creating real incentives for innovation and value creation. Following the lean paradigm, this value must be defined purely from the patients' perspective, and the entire health system needs to work towards the creation of such value. This paper offers as a practical example a lean approach to HTA, arguing that such an approach would lead to better incentives for innovation in health, as well as more patient-friendly outcomes in the long run.

ISPOR 2018 Baltimore, MD, USA


We would like to share with you the exciting activities the Global Networks Latin America team has prepared for attendees during ISPOR 2018, May 19-23, Baltimore, MD, USA. This year ISPOR 2018 will be focused on “Real-World Evidence, Digital Health, and the New Landscape for Health Decision Making.” This conference will feature three plenary sessions and over 2,100 presentations. This includes workshops, issue panels, forums, symposia, podium presentations, and poster presentations on key topics in HEOR. The pre-meeting Short Course Program is composed of 37 courses half- and full- day courses in HEOR, including 11 new course topics. ISPOR 2018 is expected to bring together 4,000 healthcare stakeholders and provide opportunities to learn, engage, and advance your career, as well as network with other professionals in health economics and outcomes research from all over the world.

A potential shift in the treatment of soft-tissue sarcoma.
Treatment with doxorubicin is a present standard of care for patients with metastatic soft-tissue sarcoma and median overall survival for those treated is 12–16 months, but few, if any, novel treatments or chemotherapy combinations have been able to improve these poor outcomes. Olaratumab is a human antiplatelet-derived growth factor receptor α monoclonal antibody that has antitumour activity in human sarcoma xenografts. The use of olaratumab with doxorubicin in patients with advanced soft-tissue sarcoma met its predefined primary endpoint for progression-free survival and achieved a highly significant improvement of 11·8 months in median overall survival, suggest a potential shift in the treatment of soft-tissue sarcoma.
