📌 Entering the Clinical Arena — Day 2 of IFAD 2026 in Antwerp

A morning built around perioperative and organ-specific fluid decisions
Friday 4 December, Day 2 of IFAD 2026 carries the theme "Entering the Clinical Arena." The opening plenary, "Fluids before and during surgery, in neurological and pulmonary disease, and in dysnatremias," provides practical guidance on perioperative fluid management and fluid therapy in pulmonary disease, with Konstanty Szuldrzinski (Poland) addressing fluid management in pulmonary disease specifically.

A second plenary extends the clinical scope
Later that day, a second plenary session, "Fluids in Hyperglycemic, Hepatic, Pancreatic and Cardiac Syndromes: When to Use Diuretics," is also part of the Day 2 programme, broadening the discussion of fluid decisions across organ systems.

💧 Perioperative Fluid Management — Individualized, Not Standardized

Surgery type and duration drive fluid balance
A perioperative clinician needs in-depth knowledge of body fluid composition, the effects of the metabolic stress response on fluid shifts, and the indications for timely de-resuscitation. The type and duration of surgery largely influence fluid balance, and understanding this relationship is pivotal to patient safety (Chapter 18, "Perioperative Fluid Management," pp. 363-378).

Moving away from one-size-fits-all fluid protocols
Years of practice focused on aggressive fluid administration without adequate checks, overlooking harmful side effects; the current approach instead individualizes fluid therapy based on need, assessed dynamically with a range of monitoring tools available to the perioperative team (Chapter 18).

🩺 Pattern Recognition, Practiced Live — SimPat at IFAD 2026

The afternoons at IFAD 2026 are built around rotation, not lecture: 11 focused small-group topics split across the two congress afternoons — six on Day 1, five on Day 2 — each running roughly 20-25 minutes so every participant works through the material directly rather than watching from a distance.

One of those rotations, the SimPat Station: Hemodynamic Shock Pattern Recognition, is led by Manu Malbrain and Mark Van Iperen, giving participants a hands-on setting to work through shock patterns as they actually present.

💧 Fluid Unresponsiveness — the Signal to Reverse Course

Not every hemodynamic assessment ends in "give more fluid." Chapter 5 of Rational Use of Intravenous Fluids in Critically Ill Patients makes the reverse case: in fluid accumulation syndrome (FAS), the finding of fluid unresponsiveness can itself serve as a trigger — and a safety parameter — to guide active fluid removal and help restore euvolemia.

This sits alongside a broader caution the chapter raises throughout: determining fluid responsiveness before administering fluids is what avoids volume overload and the added morbidity and mortality that comes with it. Assessing responsiveness isn't only about identifying who needs fluid — it's equally about identifying when to stop, and when to reverse.

📌 From the ward to the ICU — one plenary follows the patient's journey

Day 1 of IFAD 2026 in Antwerp closes its morning prescribing plenary with three sessions that trace fluid resuscitation across escalating levels of acuity.

Nicole Siparski (11:15) opens with how to read fluid balance charts — the practical skill underlying every resuscitation decision. Michelle Chew (11:30) follows with basic fluid resuscitation on the ward, and Xavier Monnet (11:45) closes the plenary with advanced fluid resuscitation in the critical care departments.

Together the three sessions move from bedside chart interpretation to ward-level resuscitation to critical-care-level decision-making — Thursday 3 December, Antwerp.
