A rare vascular variant where the left gastric artery arises directly from the abdominal aorta, with associated compression by a thickened median arcuate ligament. An important reminder that not all anatomical deviations explain symptoms — but recognizing them can prevent diagnostic confusion and guide thoughtful clinical correlation.

This is a case that had confused the surgeon, baffled the radiologist, and perhaps shocked the conference editors & reviewers.
At the minimum, the 14 yrs old girl had to undergo surgery twice, only because of the unexpected / unforeseen nature of her disease. However, meticulous scrutiny of findings & cooperation between surgeon & radiologist ultimately pinpointed the pathology, guiding proper treatment. A remarkable triumph of collaboration and determination!
