A crucial part of fluid management involves understanding when to use—and when to avoid—key intravenous (IV) solutions.
Here is a summary of the uses and contraindications for five common IV fluids:
💧 1. Normal Saline (NS)
Use: Dehydration, Hyponatremia, Wound dressing, and Vomiting electrolyte replacement.
Never Use: In patients with Heart Failure, Pulmonary edema, or Renal impairment.
🧪 2. Mannitol
Use: To increase urination, Acute renal failure, Cerebral edema, and Glaucoma.
Never Use: In Severe Kidney Diseases, Severe dehydration, Lung bleeding, or Severe heart failure.
🍬 3. DNS (Dextrose Sodium Chloride Solution)
Use: Dehydration, Hypoglycemia, and Fluid maintenance.
Never Use: In Diabetic patients, Heart failure, Kidney Patient, or Pulmonary Edema.
✨ 4. Isolyte
Use: Multiple electrolyte and dextrose injection, addressing levels of Dextrose (5\text{gm}), Sodium Acetate (0.32\text{gm}), Potassium Chloride (0.13\text{gm}), Magnesium Chloride (0.3\text{gm}), and Dipotassium hydrogen phosphate (0.026\text{gm}).
Never Use: In Hypervolemia, Renal Insufficiency, or Urinary Tract Obstruction.
⚖️ 5. Ringers Lactate (RL)
Use: Burns, Acute Blood Loss, Hypotension, Shock, and Acidosis.
Never Use: In Liver diseases or if there is an Allergy to Ringers.

🧠 Glasgow Coma Scale (GCS): Rapid Neurological Assessment
- The GCS is a standardized tool used to assess a client's level of consciousness and neurological function, especially after traumatic brain injury. It evaluates three key areas:

- Eye Opening (E) – scored 1–4
- Verbal Response (V) – scored 1–5
- Motor Response (M) – scored 1–6

➡️Total Score Range: (3–15)
- 15 = Fully alert and oriented
- ≤8 = Comatose or unconscious (consider intubation)
- 3 = Deep coma or severe impairment

• Brain Injury Classification :
- 13–15 = Mild injury
- 9–12 = Moderate injury
- 3–8 = Severe injury

💡Clinical Note: A GCS ≤8 often indicates a need for airway protection. Accurate scoring is critical in guiding care decisions and tracking neurological changes.
