অনেক সময় কাজ করতে গিয়ে time management একটি বড়ো সমস্যা হয়ে দাঁড়ায়। নিচের technique টা follow করতে পারেন।

১. কোন nerve এর compression এ এমন হয়েছে?
২. Tarsal tunner এর anterior most structure কোনটি?

আজকে আমরা এক ধরনের knee injury সম্পর্কে জানবো।

Football, tennis, basketball এ ধরনের contact sports এ aggressive twisting এর জন্য knee injury (tear of meniscus) হতে পারে।

ছবিতে এক রকম tear দেখানো আছে।

The chief symptom of this meniscal tear is pain and discomfort.

The other symptom that often accompanies this type of tear specifically is a locked knee joint. This occurs when your joint will not fully straighten after it’s bent.

আজকে আমরা একটি important anatomical relation সম্পর্কে জানবো।

আগে scenario টা দেখে নেই।

Rotator cuff injury নিয়ে ভর্তি হওয়া একজন patient এর operation চলছে।

The orthopedic surgeon exposed the muscle in the supraspinous fossa so that she could move it laterally, in repair of an injured rotator cuff.

As she reflected the muscle from its bed, an artery was exposed crossing the ligament that bridges the notch in the superior border of the scapula.

What artery was this?

A. Subscapular

Larynx এর nerve supply টা clinically অনেক important।

শুরুতে নিচের ছবিগুলো দেখো। তারপর problem টা solve করো।

55-year-old man is admitted to the emergency department with a complaint of pain when chewing over the previous 3 months.

Physical examination reveals the
patient suffers from odynophagia (painful swallowing) and some hoarseness in his speech.

Radiographic examination reveals a tumor at the tracheoesophageal groove.

Which of the following nerves is most likely affected by the tumor?

A. Recurrent laryngeal
B. Internal laryngeal
C. Vagus
D. External laryngeal
E. Phrenic

Additional info:

The innervation of the larynx is intricate, detailed, and represents some of the more complex relationships in the anatomy of the head and neck.

The vagus nerve is the large nerve that supplies the many branches of nerves that innervate the larynx.

The superior laryngeal nerve, its external and internal branches, and the recurrent laryngeal nerve all have very distinct roles in motor and sensory innervation of the larynx.

Superiorly, sensory and motor innervation are separated by internal and external branches of the superior laryngeal nerves. Inferiorly, the sensory and motor innervation is provided by the recurrent laryngeal nerve.

Damage to the recurrent laryngeal as a result of surgical intervention or the presence of a tumor in the tracheoesophageal groove would render the patient hoarse.

This hoarseness is due to a lack of innervation by the recurrent laryngeal nerve to most of the muscles of the larynx.

Damage to the internal laryngeal nerve would cause a loss of sensation above the vocal
cords, in addition to a loss of taste on the epiglottis.

Damage to the external laryngeal, which can occur during thyroidectomy, will result in a loss of innervation to the cricothyroid muscle, with resultant vocal weakness with a fatigued voice.

আজকে আমরা ছোটো একটি scenario দেখে একটি question এর answer দেবো।

একজন ১৭ বছর বয়সী মেয়ে severe dyspnea নিয়ে hospital এ ভর্তি হলো। Doctor examine করে দেখলেন তার asthma attack হয়েছে এবং bronchospasm হচ্ছে।

নিচের ছবি দেখে তোমাদের বলতে হবে, এই bronchospasm এর জন্য কোন nerve responsible?

আজকে আমরা ছোট্ট একটি বিষয় নিয়ে কথা বলবো, তা হলো peripheral nerve এর functional component।

Cranial এবং spinal nerve কি জিনিস এটা তো আমরা সবাই জানি। এদের functional component গুলো ও জানি। তাই না?

আমার দু’টি প্রশ্ন।

1. দু’টোর functional component গুলো কি same?

2. ধরো, তোমার পিঠের মাঝখানে একটি মশা কামড় দিলো। কোন ধরনের nerve fiber (functional component) এই sense কে carry করে CNS এ নিয়ে যাবে?

Masson's trichrome stain এর নাম কি আমরা শুনেছি? এটি একটি special stain।

এটি কেনো use করা হয়?

আজকে আমরা কথা বলবো vertebral column এর crush injury এর একটি কারণ bone resorption নিয়ে।

Bone resorption ব্যাপারটা কি?

The process of resorption (remodeling) involves the removal of hard bone tissue by osteoclasts followed by the laying down of new bone cells by osteoblasts.

Bone কি বয়সের সাথে ক্ষয় হয়?

Bone mass is achieved by age 25 to 30.1 After this age, there is a steady 3 to 5% rate of bone loss per decade.

Bone loss হলে সমস্যা কোথায়?

It results in a decreased number of trabecular plates, leaving a weakened architectural structure and increased bone fragility and risk of fracture (ছবিতে দেখো)।

Trabecular thinning and bone loss occurs with advancing age in both men and women,
but to a greater extent in women.

কখন female দের osteoporosis হয়ে থাকে?

At first decade after menopause, bone loss affecting the lumbar spine nearly triples in women.

এটা হয়ে থাকে estrogen কমে যাওয়ার জন্য।

Bone loss can also occur secondary to other causes, such as long-term steroid use, which accelerates bone resorption by osteoclasts.

Kyphosis এই term টার সাথে আমরা সবাই পরিচিত। তাই না?

Gibbus deformities এরই একটা ধরণ।

“Gibbus” is derived from Latin “gibbosus”, এর মানে হলো পিঠের কুজ।

এতে কি হয়ে থাকে?

It is characterized by

anterior collapse of one or more vertebral bodies resulting in kyphosis.

এটা কেনো হয়ে থাকে?

Deformities usually arise from spinal infections, and are traditionally associated with tuberculosis;

other pathogens are rarely reported in the literature.

একটি ভালো OSPE question কেমন হওয়া উচিৎ?

কতগুলো criteria থাকলে আমরা একটি OSPE কে ভালো হিসেবে গণ্য করতে পারি।

প্রথমত, একটি ভালো OSPE question অবশ্যই course objective কে address করবে।

যে practical skill কে আমরা assess করতে চাচ্ছি তা, question টি সেটাই assess করবে।

Question এর language হবে হবে grammatically correct এবং এখানে কোনো ambiguity থাকবে না।

Question টা এমন যেন না হয় যে, ছাত্ররা উত্তরটি না জেনেও শুধুমাত্র অনুমান করেই সঠিক answer দিয়ে দিতে পারছে।

অবশ্যই, এটি higher level of cognitive domain কে assess করবে।

উত্তরটা হবে ছোট; লেখা complete করা নিয়ে যেন ছাত্রদের তাড়া না থাকে। Question টা পড়ে লেখার চাইতে সঠিক উত্তর টা decide করতে যেন ছাত্রদের সময়টা বেশি ব্যয় হয়।

Question segment গুলো structured হতে হবে এবং mark allotment টা যথাযথ হতে হবে।

Answer key টা ও এমন হবে যেন তা prepare করতে কম সময় লাগে। এটি complete হতে হবে। উত্তরটাই এমন হবে যেন, marking সব examiner এর same হয়।

এখন বলছি, সবচেয়ে গুরুত্বপূর্ণ বিষয় টি। Question segment অবশ্যই supplied tool এর সাথে related হতে হবে, যেন ছাত্ররা tool টা দেখে উত্তরটা করতে পারে।
