Brain Stem Syndrome: Brain stem আমাদের CNS একটি Vital part.এর কোন অংশে Lesion হলে কি ধরনের Feature হয় সেটা আমাদের একটু জানা দরকার,,চলুন জেনে নেই সে ধরনের কিছু Lesion এর Feature.

🔶Weber Syndrome:

✅Lesion:mid brain due to infarction of
Posterior Cerebral Artery

✳Ipsilateral Side: 3rd nerve palsy
Contralateral Side:Pyramidal tract

🔶Ventral pontine Syndrome/Millard Gubler
Syndrome:

✅Lesion:Ventral part of lower end of Pons
due to infarction of pontine perforating artery
or basilar artery

✳️Ipsilateral side:7th & 8th CN
Contralateral :Pyramidal tract

🔶Lateral Pontine Syndrome/AICA Syndrome:

✅Lesion:Infero-lateral part of pons due to
infarction of Anterior-inferior cerebellar artery

✳️Ipsilateral side:
CN 7 & 8
middle cerebellar peduncle
Sympathetic trunk

Contralateral side:Spino-thalamic tract


🔶Medial Medullary Syndrome

✅Lesion:Medial part of medulla due to
infarction of anterior spinal artery or vertibral

ওয়ার্ডে সেকেন্ডই যেখানে জীবন নির্ধারণ করে Acute Medicine জানা বাধ্যতামূলক

বাংলাদেশে এখনো Acute Medicine নিয়ে কোনো আলাদা Postgraduate Degree নেই।
Acute Medicine শেখার জন্য ভালো standard book, নিয়মিত short course / training / CME—সবই খুব সীমিত।
প্রতিদিনই আমাদের ওয়ার্ডে অসংখ্য Acute Presentation নিয়ে রোগী আসছে।
একজন Acute রোগীর Outcome অনেকটাই নির্ভর করে Attending Doctor-এর দক্ষতার উপর।

Topic:OPC Poisoning
OPC(Organo Phosphorus Compound)are the organic sunstances which are derivatives of phosphoric acid.
It has bitter in taste,Clear or opaque in color & find kerosine like smell.
poisoning is common nowadays in rural areas as a suicidal poison due to--
➡Cheap
➡Easily available
➡Highly toxic
:
✅ Alkyl Compound- HETP,TEPP,Systox Dimefox
✅Aryl Compound-Parathion,Para-oxon,Chlorothion
it works?
It ⛔AChE enzyme as a result,⛔ breakdown of ACh in synapse ➡️ overactivityof ACh ➡️ Produce sign-symotoms.
OPC can absorb through all route.
It's fatal dose-
TEPP:50mg IM or 100 mg oral
Parathion & OMPA-80mg IM/175 mg oral
Fatal period: within 24 hours(untreated cases)/10 day(unsuccessfully treated cases)
are the.S/S of it?
S/S appear when cholinesterase level drops to 30% of it's normal activity.
⭕Muscarinic: potentiates post ganglionic activity.
🔷⬆all types of secrrtion in the body
🔷Miosis
🔷Bronchospasm,Vomiting, diarrohea
🔷Urinary incontinence
⭕Nicotinic: mainly paralysis of postganglionic & somatic nerve.
🔷Skeletal muscles become cramp,twisted
🔷Sympathetic ganglia-Tachycardia,hypertension
⭕CNS effect:
🔷 Depression
🔷 Drowsiness
🔷 Restlessness
🔷Ataxia
::At first-
🟨 Remove patient from source
🟨 Remove his cloth & wash with soap & water
🟨 Gastric lavage:1:5000 KMnO4 or Activate charcoal(1g/kg)
🟨 Give -Antidote as::
↪️Atropine: start with 2-4 mg IV & repeated every 5-10 min until atropinization
↪️Pralidoxime-act as a specific cholino-esterase reactivator.
