Celebration of Dentist Day 2019

This post is on the request of some MS Residents of the Dept of Prosthodontics, BSMMU. They wanted to know about Bone Grafting & Autogenous Platelet Rich Plasma (PRP) what I use in Implant Surgery to enhance Osseointegration. So, let's have a look at bone grafting and later I will try to discuss about the PRP.

BONE GRAFTING
Bone grafting is a surgical procedure in order to replace and repair lost and inadequate bone. Most bone grafts are expected to be reabsorbed and replaced with natural bone over a period of time resulting in a fully integrated region of new bone.

According to source, bone grafts may be AUTOLOGOUS (harvested from the patient’s own body), ALLOGRAFT (cadaveric bone stored in bone bank), SYNTHETIC (made of hydroxyapatite/naturally occurring biocompatible substances), or XENOGRAFTS (originated from a species other than human, such as bovine bone or porcine bone).

The biologic mechanisms/principles involved in successful bone grafts include OSTEOCONDUCTION (guides the growth of natural bone), OSTEOINDUCTION (converts undifferentiated cells to active osteoblasts), OSTEOPROMOTION (enhances osteoinduction) and OSTEOGENESIS (graft material itself contributes to bone remodeling). Osteogenesis only occurs with autografts.

Reasons that bone grafting might be needed in dentistry include reconstruction of surgical defects, sinus augmentation, socket preservation, ridge augmentation or regeneration, placement of dental implant.

A standard surgical procedure is followed for bone grafting with the slight variation that the situation demands.
