Supraventricular tachycardia (SVT) তে Carotid sinus massage একটি Treatment option হলেও, Ventricular tachycardia (VT) তে Carotid sinus massage এর কোন role নেই কেন?

Carotid sinus massage দেওয়া মানে vagal stimuli দেওয়া, যাতে Heart rate কমে যায়।
SVT কিংবা VT কোনটাতেই cardiac impulse SA node থেকে আসেনা। SVT তে atrial muscle থেকে এবং VT তে ventricular muscle থেকে high rate এ abnormal impulse আসে।

Atrial muscle supplied by both sympathetic and parasympathetic nerve, but ventricular muscle is supplied only by sympathetic nerve.

Ventricle এ যেহেতু Vagal supplyই নেই, তাই VT এর heart rate carotid sinus massage দিয়ে কমানোর প্রশ্নই আসে না

As a rule,Skeletal muscle fibres do not branch. Branching however, takes place in the muscle of the tongue.

A 55 years old man, BMI- 25, recently diagnosed with DM with no family history of diabetes, presented with upper abdominal discomfort.

Which disease should be suspected?

Spinothalamic tract axons ascend to several regions of the thalamus. There is
tremendous divergence of the pain signal from these thalamic sites to several
distinct areas of the cerebral cortex that subserve different aspects of the pain
experience .

One of the thalamic projections is to the somatosensory cortex. This projection mediates the purely sensory aspects of pain, i.e., its location, intensity, and quality.

Other thalamic neurons project to cortical regions that are linked to emotional responses, such as the cingulate gyrus and other areas of the frontal lobes, including the insular cortex.

These pathways to the frontal cortex subserve
the affective or unpleasant emotional dimension of pain. This affective dimension
of pain produces suffering and exerts potent control of behavior. Because of this
dimension, fear is a constant companion of pain. As a consequence, injury or surgical lesions to areas of the frontal cortex can diminish the emotional impact of pain while largely preserving the individual’s ability to recognize noxious stimuli as painful.

Intestinal parasite infection, such as hookworm, may be associated with a reduced risk of asthma.

আমরা জানি, Viscera র nerve supply autonomic. কিন্তু ishchemia, spasm, overdistension এর কারনে visceral nerve ending stimulated হয়ে pain হতে পারে।

But a few visceral area are almost completely insensitive to pain of any type. এরকম কিছু ভিসেরা হচ্ছে-

> Liver parenchyma
> Lung alveoli

But remember, liver capsule, bile duct, bronchi are highly sensitive to pain.

Mechanism of Hypokalaemia and Hyperglycemia in Cushing Syndrome:

Cortisol can activate aldosterone receptors, but it does not normally do so. Because most cells containing aldosterone receptors also express an enzyme called 11beta- hydroxysteroid dehydrogenase type 2 (11B-HSD2), which inactivate cortisol by converting it to cortisone.

But in Cushing syndrome, the high cortisol level overcome the capacity of 11B-HSD2 to inactivate cortisol in kidney resulting in activation of aldosterone receptors.

Aldosterone receptor active হওয়ার কারনে কিডনি বেশি বেশি Potassium বের করে দেয় যার কারনে Hypokalaemic alkalosis হয়। এই Hypokalaemia র জন্য আবার Insulin সিক্রেশন কমে যায় যেখান থেকে Hyperglycemia হয়।

One major difference between bacterial DNA & eukaryotic DNA is that bacterial DNA has no introns, whereas eukaryotic DNA has both introns & exons.

কীভাবে Chronic Disease এ anemia হয়?

প্রথমত প্রয়োজনীয় হিমোগ্লোবিন তৈরির জন্য রক্তে একটি নির্দিষ্ট পরিমান iron থাকতে হয়। রক্তের এই নির্দিষ্ট পরিমান iron আসে প্রধানত Small intestinal cell এবং Reticuloendothelial cell ( such as macrophage) থেকে। এসব cell এর plasma membrane এ 'ferroportin' নামক এক প্রকার exporter থাকে। এই ferroportin দিয়ে কোষের ভিতরের iron রক্তে আসে।

Chronic inflammatory কন্ডিশনে pro-inflammatory cytokine ( IL-1, IL-6, TNF) continuously তৈরি হতে থাকে। IL-6 এর প্রভাবে liver অতিরিক্ত পরিমানে 'Hepcidin' নামক একটি protein তৈরি করে। এই hepcidin তখন intestinal cell এবং macrophage membrane এ বিদ্যমান iron exporter 'ferroportin' এর সাথে bind করে একে cell এর ভিতরে নিয়ে যায়।

Due to lack of ferroportin, iron cannot be exported to blood and get trapped inside these cells. Ultimately, low serum iron leads to anemia.

রক্তে lipid বেশি থাকলে MCV কেন বেশি হয়?

Vitamin B12 অথবা Folate deficiency ছাড়াও PBF এ RBC macrocytosis পাওয়া যেতে পারে। এরকম কতগুলো কারন হল - liver disease
hypothyroidism
hyperlipidaemia
pregnancy.
সবগুলো কন্ডিশনেই রক্তে lipid বেশি থাকে।

RBC membrane এর লিপিড বাইলেয়ার একটু আলাদা বৈশিষ্ট্য সম্পন্ন। এটি plasma lipid এর সাথে নিজের lipid exchange করতে পারে। সেজন্য রক্তে lipid বেড়ে গেলে RBC membrane এও তা বেশি করে জমা হয়। যার ফলে MCV বেড়ে যায়।

বাইরের কোন স্নায়ু উদ্দীপনা ছাড়াই SA Node কীভাবে Cardiac impulse তৈরি করে?

SA Nodal fibers are naturally leaky to Na ion and Ca ion. যেহেতু SA Node এর resting membrane potential খুব কমঃ -55 to -60 mV.
So in between heart beat, positive Na ion leaky Na channel দিয়ে বাইরে থেকে SA node এর ভিতরে প্রবেশ করে। ফলে SA node এর ভিতরের Negativity কমতে কমতে যখন threshold level (-40 mV) এ পৌছায় তখন Action potential শুরু হয়ে যায়।

Therefore, basically the inherent leakiness of the SA nodal fibers to Na and Ca ions causes their self excitation.
