Hey everyone,

The answer to last weekend's trivia question is D-Upwards movement of the ipsilateral diaphragm, downwards movement of the contralateral diaphragm. This is known as paradoxical movement of the diaphragm and occurs because on the paralyzed side, the diaphragm is mechanically sucked upwards by the reduced intrathoracic pressure upon inspiration.

Hey everyone,

The answer to last weekend's trivia question is B. Prednisone PO (or another oral steroid). The Vicks rub has been aspirated and caused an exogenous lipoid pneumonia which symptomatically resembles a viral or atypical bacterial pneumonia. The treatment is therefore aimed at removing the offending agent Vicks, and reducing the resulting inflammation.

Hello everyone,

The correct answer to this weekend's trivia question is A-Low Tidal Volume (6ml/kg), High PEEP (10-20 cm H2O). The reason for this is that in ARDS there is a non-uniform decrease in the lung compliance in certain areas, and as such air is shunted preferentially to more compliant areas. If the tidal volume is kept too high, there is a risk of barotrauma from overinflation of the more compliant areas. Likewise, the high PEEP helps to prevent alveolar collapse and maintains alveolar surface area, thus aiding proper oxygenation and preventing trauma from repeated cycles of atelectasis and reopening. The answer by Armand Gumera sums this up perfectly!
