When I was a classroom primary teacher, I found that providing a basic feedback form like this one, was really helpful, for when I had visiting or relief teachers come into my class while I was away. It meant I could have it out ready for them to fill in through the day and communicate any potential issues to follow up, or praise for how well the class behaved, or what they achieved in the day. Likewise, in my relief teaching days, I made copies of these and always had them with me for each Relief teaching day. The teachers I left these for were always grateful and loved the insight into the day they were away.
