Our patient cases present a real-world patient scenario and accompanying learning activities.

Here is an overview of the case we will be presenting in our Musculoskeletal course (lumbar spine) on Saturday 18 March.

"Vanessa Hayes is a 41 year old woman who has had back pain for many years. She usually manages it well, but it does stop her doing some of the things in her life that she used to enjoy. However, she recently experienced a flare up that was different to all others that she has had. She was taken to the emergency department, and required a stint in the inpatient rehabilitation ward. She is presenting today to physiotherapy outpatients for a review appointment."

How confident are you in providing evidence-based interventions for people with low back pain?

Good news! In September 2022 the "Low Back Pain Clinical Care Standard" was published by the Australian Commission on Safety and Quality in Health Care.

Eight "quality statements" were made that describe the care that should be provided to people with low back pain.

Included were evidence-based guidelines for clinicians on:

- clinical assessment (interview and physical examination, including red flags, referrals and imaging)
- psychosocial assessment
- patient education and advice
- self-management
- physical interventions
- pain medicines
- review and referral

For physiotherapists, your decisions regarding which of these to include is based on your assessment (information review, interview, and physical examination) in which you would have determined the impairments, activity limitations, and participation restrictions of the patient.

There are some great examples here of exercises you could use with your patient combined with education on pacing and graded activity.

We have had a few questions from people about post-mastectomy Physiotherapy management. I thought I would share this resource from the Chris O’Brien LifeHouse in Sydney, one of Australia’s leading Cancer Centres.

As you can see, the patient education guide refers to instructions from surgeons and physios AND suggests that there are different procedures that are managed in different ways. Always check the surgeons orders and protocols. But here are some ideas for you on directions you could take

Anita Spiteri is a 44 year old woman who started feeling pain in her right shoulder about 4 weeks ago. It has progressively worsened across this time. She does not remember doing anything to injure her shoulder. She went to her GP 4 days ago, who referred her for an MRI scan and to see a physiotherapist. She is presenting today to physiotherapy outpatients for a review appointment.

Stuart Potter is a 82-year-old retiree who fell at home last week as he was getting out of the shower. He was taken to the Emergency Department via Ambulance and after an orthopaedic consult was diagnosed with a wrist fracture. A GAMP procedure occurred the following day. He was transferred to the rehabilitation ward 1 week after the surgery, and is now awaiting review by the rehabilitation ward physiotherapist.

What factors would influence whether this patient can be discharged home?

Cardiac Surgery

Mrs Cynthia Yang, a 58year-old woman, was admitted yesterday for coronary artery by-pass surgery. Six weeks ago, the patient experienced crushing chest pain that radiated into their jaw and left arm and presented to the ED. The patient was diagnosed as myocardial infarction and discharged home five days later. After stabilising, and review by the cardiac surgeon four weeks ago, a decision for CABG surgery was made and the patient placed on a waiting list. The surgery was completed two days ago.
