Here's why most rail investigation findings never lead to structural change.

The ATSB's 2024-25 Annual Report puts a number on it.

90 safety issues were identified across Australian transport investigations last year, covering rail, aviation and marine.

Only 57% were addressed through appropriate safety action.
A further 10% were partially addressed.

That leaves roughly a third unresolved.

The investigations surfaced the issues. The findings made it into the reports. But the path from finding to corrective action broke down somewhere along the way.

The ATSB had to issue 19 formal safety recommendations. These are escalations that only happen when the responsible parties haven't acted on their own.

In rail specifically, one investigation into a Brisbane SPAD took over two years to complete. The finding was a system design issue where routine alerts had become so habitual that a driver acknowledged one without processing it.

That kind of finding only surfaces when the investigation method goes deeper than "who made the mistake.“

3 things tend to break the chain between finding and fix.
→ The corrective action is too vague to implement. "Improve communication" doesn't give anyone a next step.
→ The finding addresses the individual, not the system. Retraining one person doesn't change the conditions that created the problem.
→ The investigation took so long that the context has shifted. New team, new priorities, same unresolved risk.

The question worth asking isn't whether your team can investigate. It's whether your investigation method consistently produces findings that lead to structural change.

This is one of the most common reasons audits feel harder than they should.

It’s not that teams don’t have material.
It’s that the material doesn’t answer the question the audit is actually asking:
“Show me it’s working.”

A useful way to pressure-test your own pack before anyone else does:
➡️ Can someone independent follow the decision pathway from risk → control → implementation → check → outcome?
➡️ Can you show consistency, or does it depend on who’s on shift / which site you’re at?
➡️ Can you retrieve proof fast, or does it turn into a hunt across folders, inboxes, and people?
➡️ Does the evidence show verification, or only that something was written or delivered?

If you can’t answer those cleanly, you usually end up doing “audit prep” that’s really just rebuilding the story.

Paper systems look solid until the pressure's actually on.
