How a clinician comes across in a clinical interaction may have biological consequences.
A 2017 Stanford study (Howe, Goyer and Crum) tested this directly. Participants were pricked with histamine to trigger an allergic skin reaction, then had an inert cream applied.
Some were told the cream would calm the reaction, others that it would worsen it. The clinician applying the cream was either warm and competent, or cold and unsure.
When the clinician was cold and unsure, what people were told about the cream made no measurable difference to the wheal that formed. When the clinician was warm and competent, those told the cream would calm the reaction had a noticeably smaller wheal.
Findings like these are a nice way into a bigger idea: how we show up with patients can be part of what their bodies are responding to, not just a backdrop to the treatment itself.

Chilli feels hot. Mint feels cool. But neither one changes your body temperature.
The capsaicin in chilli opens TRPV1, the same ionotropic channel that opens when your skin actually gets hot. The menthol in mint opens TRPM8, the channel that opens when your skin is genuinely cool.
When these channels open, positively charged ions flow across the cell membrane, which can depolarise the neuron enough to fire an action potential, the electrical signal sent along the nerve.
So the input heading towards the dorsal horn and brain looks much like the input from real heat or cool, even though nothing thermal has happened.
Appreciating the physiology behind sensations like these is a nice way into a bigger idea: our experiences are shaped by a complex array of factors, not a direct readout of what's happening in the tissue.
