Never assume graft dysfunction is rejection until inflow is properly assessed.
Renal transplant recipient with progressive creatinine rise. Doppler showed high resistive indices and pulsus parvus et tardus. MRA: patent transplant renal artery but moderate-to-severe ostial right CIA stenosis.
Catheter angiography + IVUS + pressure gradient: 30 mmHg systolic gradient across the CIA, zero gradient across the transplant artery. IVUS confirmed widely patent 6.8 mm transplant artery.
Bilateral kissing stents restored inflow. Creatinine improved, Doppler normalised, planned allograft biopsy cancelled.
Key message: Actively exclude arterial inflow stenosis in unexplained transplant dysfunction.

🎓 Expanding access to IR education across Eastern and South-eastern Europe!
CIRSE’s new Access to Care Initiative reached its first milestone at the ESIR course in Maribor, where 15 young European IRs received funding through the Gehrig-Pereira Grant to take part in expert-led, hands-on training in acute limb ischaemia.
From practical skills to new professional connections, the initiative is helping make high-quality IR education more accessible to the next generation.

Use of intravascular lithotripsy for a heavily calcified, near-flush thrombotic occlusion of a celiomesenteric trunk.
Elderly patient with acute-on-chronic mesenteric ischaemia. Shared celiac-SMA origin with new thrombus + dense calcium. Left radial access, sequential wire escalation to Astato (20g tip load), pre-dilatation requiring IVL to efface, finished with a drug-eluting stent.
Result: Restored antegrade flow to both celiac axis and SMA. No dissection, perforation or embolisation.
Celiomesenteric trunk disease is unforgiving — one lesion threatens both foregut and midgut. Intra-vascular lithotripsy (Shockwave) was essential for vessel preparation.

Advancing image-guided intervention at TTSH.
An AI-powered navigation and 3D treatment planning platform (CAS-One IR) is now live in TTSH for minimally invasive tumour ablation. Accurate planning and needle placement improve procedural confidence, patient safety, and treatment options in complex cases — especially those with difficult trajectories or proximity to critical structures.
Technologies like this are shaping the next phase of interventional oncology.

Patient with recurrent nasopharyngeal carcinoma. Multiple lines of systemic therapy exhausted; now on best supportive care. Extensive local recurrence causing left nasal blockage. Clear patient wish: to breathe through both nostrils again.
Natural or***ce transluminal ablation performed. At 6 weeks the nasal cavity was clear and he could breathe through the nostril. Small intervention, meaningful quality-of-life gain. These are the cases that remind us why palliative IR matters.

🏥 Tan Tock Seng Hospital is the first hospital in Singapore to offer interventional magnetic resonance imaging (iMRI) guidance, starting 2025.
iMRI lets us target lesions seen only on MRI and monitor ablation zones with real-time thermography — all without ionising radiation. This is particularly valuable in interventional oncology cases where soft-tissue contrast is critical. A meaningful step forward for precision

🏥 TTSH is one of Singapore’s largest multidisciplinary hospitals, an anchor of NHG Health, and the centrepiece of HealthCity Novena — Singapore’s first integrated healthcare hub spanning 17 hectares. We run more than 2,000 beds and deliver the full spectrum of interventional radiology. Alongside our exceptional nursing and radiography staff, we perform >12,000 cases per year.
