A Permit to Work System or PTW is a standard operational procedure used by organizations to issue documented permission to perform tasks that are considered hazardous or non-routine.

A permit to work form consists of specific instructions of the nature of the job, the time and place along with adequate information of critical safety procedures to follow.
A typical work permit consist of following information and sections:
1. Permit title
2. Permit reference number
3. Job Location
4. Plant Identification
5. Description of work to be done and its limitations
6. Hazard identification
7. Precautions necessary and action in the event of an emergency – 8. people who carried out precautions.
9. Protective equipment
10. Issue – signature (issuing authority) confirming that precautions have been taken and any isolations have been made. Date, time and duration of the permit.
11. Acceptance – signature(S) confirming understanding of work to be done, hazards involved, and precautions required. Also confirming that permit information has been explained to all permit users.
12. Extension/shift handover procedures – signatures confirming checks made that plant remains safe to be worked on and new authorities and permit users are made fully aware of the hazards and the precautions. A new expiry time is given.

Heat Stress;
Workers who are exposed to extreme heat or work in hot environments may be at risk of heat stress. Exposure to extreme heat can result in occupational illnesses and injuries. Heat stress can result in heat stroke, heat exhaustion, heat cramps, or heat rashes.
Causes:
High air temperatures, radiant heat sources, high humidity, direct physical contact with hot objects, and strenuous physical activities.
Prevention.
1. Wear loose fitting, lightweight clothing.
2. Protect against sunburn.
3. Drink plenty of fluids.
4. Be careful with certain medicines.
5. Never leave anyone in a parked car.
6. Take it easy during the hottest parts of the day.
7. Get acclimated.
8.Be cautious if you're at increased risk.

The Fire Prevention Triangle - Heat, Oxygen and Fuel

A fire needs three elements - heat, oxygen and fuel. Without heat, oxygen and fuel a fire will not start or spread. A key strategy to prevent fire is to remove one or more of heat, oxygen or fuel. The risk assessment should include detail on all three elements to minimise the risk of a fire starting/ spreading.

The formal process of identifying preventive and protective measures by evaluating the risks arising from
a hazard, taking into account the adequacy of any existing controls, and deciding whether or not the risk is
acceptable.
Following are the 5 Step to carry out risk assessment:
1) Identity Hazard
2) Identity People who might be harm and how
3) Evaluate the risk and decide on precautions
4) Record finding and implement it
5) Review risk assessment and update accordingly
